What do I need to do to renew my facility credential?

The renewal notice you receive indicates the total amount due for your facility. The department sends notices about 45-60 days before the license expiration date.

This is a courtesy and isn't required for renewing a credential. If you have a notice, please use it because this is the most efficient and accurate way to renew. To renew without a notice, send the payment with your name, credential number, current mailing address and documentation of any other requirements to:

What do I need to renew my in-home services agency license?

In-home services (IHS) agencies include home care agencies, home health agencies, hospice agencies and hospice care centers. Please see each agency website for application packets.

  • Complete an application.
  • Pay a fee (WAC 246-335-990).
  • Ensure the Washington State background checks are current (within the last three months) for the on-site administrator, director of clinical services (home health or hospice) or the supervisor of direct care services (home care).
  • Send disclosure statements signed by the on-site administrator, director of clinical services (home health or hospice) or the supervisor of direct care (home care). You may find a sample disclosure statement in the application packet.
  • Send proof of professional liability insurance for $100,000 per occurrence, and public liability and property damage insurance in the amount of $200,000 per occurrence.
  • Send a copy of the master business license. You can get this from Department of Revenue.

How long will it take to process my renewal?
  • By Mail: It takes about two weeks to update. This includes mailing time and receipt of all required documentation.
  • In Person: If you deliver the payment and all required documentation in person to the front counter it'll take about seven business days.

How do I renew my credential without a renewal notice?

Courtesy renewal notices aren't required for renewal. You may update your credential by mailing your renewal payment to us with documentation of your name, credential number, current mailing address and any other renewal requirements. You may renew in person without a renewal notice at our Tumwater location.

What is my expiration date?

Your credential and renewal notice list the expiration date. You may also find your expiration date on our provider/facility search. Your credential expires at midnight on your expiration date.

How do I request a duplicate of my credential?

Please email requests for a duplicate credential, and remember to include your credential number. If it's within 90 days of your expiration date, you may renew your credential and receive an updated copy. Submit requests to our credentialing review staff members.

How do I update my mailing address?

You may submit your mailing address change requests in writing. If you're renewing and have received your renewal notice, complete the address change request on the back of the card. You may fax your mailing address change to 360-236-4918, or email it to our Credentialing staff. We'll mail your updated credential to the address on file. The U.S. Postal Service will not forward state mail so it's your responsibility to inform the Department of Health of any address change.
