State disciplines health care providers           

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Benton County

In August 2016 the Pharmacy Commission charged pharmacy technician Dana M. Linder-Michael (VA00058481) with unprofessional conduct. Linder-Michael allegedly prepared prescriptions and compounded prescription mouthwash without supervision from a licensed pharmacist.

Clark County

In August 2016 the secretary of health conditionally granted a registered nursing assistant credential to Jessica Ruth Duong (NA60666386) and placed her on probation for at least three years. Duong’s Washington pharmacy technician credential was suspended in 2010 after her Oregon pharmacy technician credential was suspended in 2009 in connection with allegations of diverting cash and falsifying pharmacy records.

Cowlitz County

In August 2016 the Nursing Assistant Program charged certified nursing assistant Jessica Renee Posadas Sanchez (NC60086538) with unprofessional conduct. In 2011 Sanchez was convicted of two counts of second-degree theft. In 2012 and again in 2014 she was convicted of third-degree theft.

Grays Harbor County

In August 2016 the secretary of health ended probation for chemical dependency professional trainee Chrystal Beckley (CO60553693).

Island County

In August 2016 the Nursing Commission ended probation for registered nurse Lynn Allison Paddock (RN00082701).

King County

In August 2016 the Medical Commission ended an agreement with physician Michael C. Brown (MD00028042) that placed him on probation and limited his ability to prescribe opioids.

In August 2016 the Unlicensed Practice Program entered an agreement with Mei Liu that requires Liu to cease and desist from practicing massage without a license, and pay a $500 fine. Liu doesn’t have a massage practitioner credential, but owns a business that advertised massage.

In August 2016 the secretary of health ordered Aiqin Jiang to cease and desist from practicing massage without a license, and pay a $1,000 fine. Jiang has no massage practitioner license, but owns a business that advertised massage and had no employee on site with a massage license.

In August 2016 the Nursing Assistant Program charged registered nursing assistant Sierra Denise Langford (NA60396016) with unprofessional conduct. In 2014 Langford was convicted of fourth-degree assault – domestic violence.

In August 2016 the Nursing Assistant Program charged certified nursing assistant Louise D. Brunner (NC60252823) with unprofessional conduct. Charges say Brunner didn’t respond to an order to undergo a mental examination.

Pierce County

In August 2016 the Unlicensed Practice Program entered an agreement with Leiday Aguero, also known as Leidy Aguero, that requires her to cease and desist from practicing massage without a license, and pay a $1,000 fine. Aguero advertised massage services despite not having a massage practitioner license.

In August 2016 the Nursing Assistant Program charged certified nursing assistant Semira Ahmed Hussien (NC10029618) with unprofessional conduct. Charges say Hussein didn’t respond to a Department of Health inquiry about a complaint.

In August 2016 the Nursing Commission ended probation for advanced registered nurse practitioner Susan Ann King (AP60439687).

In August 2016 the Nursing Commission ended probation for registered nurse Wendy Lee Wincewicz (RN00092329).

In August 2016 the Nursing Commission filed an amended statement of charges against licensed practical nurse Rachel K. Kletzker (LP00044669) to modify allegations involving drug use.

Snohomish County

In August 2016 the Nursing Commission agreed to reinstate the registered nurse credential of Rose F. Huggett (RN00163533) and placed her on probation. Her license was suspended in 2010 after she tested positive for cocaine, codeine and morphine.

In August 2016 the Nursing Commission ended probation for registered nurse Cindy Lea Otness (RN00114305).

Spokane County

In August 2016 the secretary of health conditionally granted a chemical dependency professional trainee credential to Cory Lane Bromley (CO60632489), placed him on probation, directed him to complete continuing education in professional law and ethics, and ordered him to undergo evaluation for a substance abuse monitoring program. In 2004 Bromley was convicted in Montana of driving while intoxicated. In 2013 he was convicted of two counts of third-degree assault. In 2014 his registered nursing assistant credential was suspended.

Thurston County

In August 2016 the Nursing Commission placed licensed practical nurse Kelly Lynn Krocker (LP00044713) on probation for at least two years, ordered her to undergo a mental evaluation, and directed her to complete continuing education in documentation and in patient assessment. Krocker submitted two assessment sheets for the same patient, with one of the sheets done from memory without using notes. The sheets differed significantly. Krocker said she had forgotten having submitted the first sheet because of a neurological condition.

Whatcom County

In August 2016 the Nursing Commission agreed to reinstate the registered nurse credential of Tina M. Brightwell (RN00069951) and placed her on probation. Her license was suspended in 2015 after she administered 10 times the amount of morphine authorized in a physician’s order to a patient at a long-term care facility, killing the patient. This incident occurred less than two months after Brightwell was placed on probation following another resident’s death where nursing errors were a major contributing factor.

Out of State

Idaho: In August 2016 the Nursing Commission ended probation for registered nurse Maribeth K. Campbell (RN00150817).

Idaho: In August 2016 the Dental Commission charged dental assistant Jeanne Heuer (D160046969) with unprofessional conduct. Heuer allegedly gave amoxicillin, both with and without a dentist’s instructions, to a patient who was allergic to penicillin. Heuer allegedly didn’t note the allergy from the patient’s chart and didn’t call it to the dentist’s attention. Charges say the patient later died.

New Jersey: In August 2016 the Nursing Commission ended probation for registered nurse Edith Tsungayi Reber-Madenyika (RN60206727).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
