OLYMPIA – The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nursing assistants, counselors).
Information about health care providers is on the agency website. View the “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Benton County
In May 2026 the Department of Health indefinitely suspended Arielle Green’s (NA61382287) registered nursing assistant credential by default order. In April 2024, the Department of Social and Health Services (DSHS) determined that Green mentally abused a vulnerable adult. The finding became final in February 2025, and Green was placed on the DSHS vulnerable adult abuse registry, effective April 22, 2024. Green also failed to respond to the statement of charges.
King County
In May 2026 the Department of Health indefinitely suspended Erin Saena Yaseen’s (NU60029155) nutritionist credential by default order. In February 2025, the department received a complaint alleging that Yaseen committed insurance fraud. In August 2025, Yaseen failed to respond to the department’s request for information and later failed to respond to the statement of charges.
Pierce County
In June 2026 the Department of Health indefinitely suspended Georgia D. Germunson’s (HM61357106) home care aide credential by default order. In April and May 2025, Germunson allegedly appeared intoxicated while caring for a vulnerable adult on multiple occasions. In June 2025, the client reported that Germunson was frequently intoxicated and requested a new caregiver. Germunson also failed to respond to the department’s investigation and its statement of charges.
In June 2026 the Department of Health charged registered and certified nursing assistant Aundrea Tyler Shaw (NC61544061, NA60756847) with unprofessional conduct. In November 2025, the Department of Social and Health Services (DSHS) determined that Shaw financially exploited a vulnerable adult. The DSHS finding became final in March 2026, and Shaw was placed on the DSHS vulnerable adult abuse registry, effective Nov. 17, 2025.
Snohomish County
In May 2026 the Department of Health charged certified nursing assistant Andrea Michelle Naylor (NC60743808) with unprofessional conduct. In July 2025, the Department of Social and Health Services (DSHS) determined that Naylor financially exploited a vulnerable adult. The DSHS finding became final in August 2025, and Naylor was placed on the DSHS vulnerable adult abuse registry, effective July 17, 2025.
Spokane County
In June 2026 the Pharmacy Quality Assurance Commission and Sandra Lee Symington Bang (PH00021680) entered an agreed order requiring Bang to surrender her pharmacist credential, permanently prohibiting her from practicing pharmacy in Washington. Between June 2022 and December 2024, Bang fabricated prescriptions for fictitious patients, filled the prescriptions and took the controlled medications for personal use.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
