State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Adams County
In October 2016 the secretary of health suspended the registered nursing assistant credential of Sarah N. Stromberg (NA60049391) for at least 30 months. In 2016 Stromberg pleaded guilty to unlawful possession of a legend drug. She diverted medication from a resident of an adult family home where she worked.
Benton County
In October 2016 the Pharmacy Commission indefinitely suspended the pharmacy assistant credential of Marianne Julia Folk (VB60286073). Folk signed a drug court agreement on a count of first-degree theft after admitting to diverting thousands of controlled substance tablets from the pharmacy where she worked.
Clark County
In October 2016 the Nursing Assistant Program charged registered nursing assistant Emilie Sue Anne Miller (NA60460906) with unprofessional conduct and immediately suspended her credential. After finding that Miller neglected a vulnerable adult, the state Department of Social and Health Services will not allow her to be employed in caring for or having unsupervised access to vulnerable adults.
Grays Harbor County
In September 2016 the Pharmacy Commission suspended the pharmacy technician credential of Holly A. Walker (VA00044504) for at least one year. Walker admitted to taking controlled substances for which she didn’t have a valid prescription.
King County
In September 2016 the Nursing Assistant Program suspended the certified nursing assistant credential of Latrina D. McNair (NC60088701) for at least five years. In 2015 McNair was convicted of third-degree assault of a child. She didn’t respond to a Department of Health inquiry.
In October 2016 the Naturopathy Board entered an agreement with naturopathic physician Mark C. Monwai (NT00001107) that suspends his credential for at least five years. In 2015 Monwai pleaded guilty to two counts of fraudulent insurance claims. Those charges were connected with his billing practices. Monwai also practiced with a suspended license, and allowed an unlicensed phlebotomist to practice at his clinic.
Lewis County
In October 2016 the Nursing Commission suspended the registered nurse credential of Loraine B. Smith (RN00161952) for at least three years. While her license was suspended, Smith worked as a nurse.
Okanogan County
In October 2016 the Nursing Commission filed an amended statement of charges against registered nurse Cory R. Riehart (RN60029502) and immediately suspended her credential. Riehart didn’t comply with a substance abuse monitoring program.
Pend Oreille County
In October 2016 the secretary of health indefinitely suspended the paramedic credential of Joshua Hicks (ES60310144). In 2016 Hicks was convicted of reckless driving, third-degree theft, and possession of legend drugs. Hicks stole controlled substances from the fire department where he worked, and admitted to being under the influence of prescription drugs when he was driving a fire department vehicle involved in an accident.
Pierce County
In October 2016 the Nursing Commission indefinitely suspended the licensed practical nurse credential of Jennifer Kay Schliesman (LP60286995), who didn’t comply with a stipulation requiring her to report her employment status.
Skagit County
In October 2016 the Nursing Commission suspended the registered nurse credential of Susan M. Tekola (RN00110993), who didn’t respond to charges of being unable to practice with reasonable skill and safety due to concerns related to mental health issues.
Spokane County
In October 2016 the Nursing Assistant Program charged certified nursing assistant Nancy A. Foreman (NC10064739) with unprofessional conduct and immediately suspended her credential. Upon finding that Foreman abused a vulnerable adult, the state Department of Social and Health Services will not allow her to be employed in caring for or having unsupervised access to vulnerable adults.
In October 2016 the Nursing Assistant Program charged registered nursing assistant Erica Kay Carson (NA00115031) with unprofessional conduct and immediately suspended her credential. Carson borrowed $20 from a patient and didn’t repay it. The state Department of Social and Health Services found that Carson financially exploited a vulnerable adult.
In October 2016 the Pharmacy Commission indefinitely suspended the registered nursing assistant credential of Susan L. Sebesta (NA00095117). Sebesta allegedly financially exploited a vulnerable adult and tested positive for methamphetamine.
In October 2016 the secretary of health indefinitely suspended the registered nursing assistant credential of Jeanette F. Pringle (NA00020633). After finding that Pringle neglected a vulnerable adult, the state Department of Social and Health Services will not allow her to be employed in caring for or having unsupervised access to vulnerable adults.
Whatcom County
In October 2016 the Chemical Dependency Professional Program entered an agreement with chemical dependency professional R Russ (CP00000957) that indefinitely suspends his credential. Russ failed an ethics program.
Yakima County
In October 2016 the Nursing Assistant Program charged registered nursing assistant Amber Lyn Wilson (NA00121573) with unprofessional conduct and immediately suspended her credential. Wilson borrowed $20 from a patient and didn’t repay it. After determining that Wilson financially exploited a vulnerable adult, the state Department of Social and Health Services will not allow her to be employed in caring for or having unsupervised access to vulnerable adults.
Out of State
Alaska: In October 2016 the Medical Commission entered an agreement with physician assistant Michael J. Niemi (PA60613099) under which he surrendered his credential. Niemi violated a monitoring contract.
Mississippi: In October 2016 the Nursing Commission charged registered nurse Michael Roy Chase (RN00140049) with unprofessional conduct and immediately suspended his credential. Chase relinquished his Colorado nursing license in 2016 after pleading guilty in that state to third-degree assault — at-risk victim, in connection with an incident involving a patient.
Missouri: In October 2016 the Nursing Commission indefinitely suspended the registered nurse credential of Carrie Lynn Vest (RN00176628). Her Alaska nursing license was revoked in 2016.
New Jersey: In October 2016 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Jean Michelle Wilson (RN60539873, AP60539874) with unprofessional conduct and immediately suspended her credentials. In 2016 Wilson surrendered her Oregon registered nurse and advanced registered nurse practitioner licenses.
New Mexico: In October 2016 the Medical Commission entered an agreement with physician David L. Dickerman (MD00036697) that indefinitely suspends his credential. Dickerman didn’t comply with a substance abuse monitoring contract.
Oregon: In October 2016 the Midwifery Program charged midwife Adele Rose (MW00000242) with unprofessional conduct and immediately suspended her credential. Rose’s Oregon midwife credential was suspended in 2016.
Wyoming: In October 2016 the Medical Commission indefinitely suspended physician James Gordon Douglas (MD00019266). His Wyoming medical license was suspended in 2015 after he didn’t abide by a monitoring agreement.
