State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In October 2019 the Surgical Technologist and Medical Assistant programs indefinitely suspended the surgical technologist, medical assistant-phlebotomist, and certified medical assistant credentials of Angela Marie Broders (ST60171520, PC60379751, CM60379757), who didn’t comply with a substance abuse monitoring contract.
Chelan County
In October 2019 the Chiropractic Commission entered an agreement with chiropractor Christie Marie Collier (CH60186568) that indefinitely suspends her credential. Collier didn’t comply with a substance abuse monitoring contract.
King County
In October 2019 the Nursing Assistant Program charged registered nursing assistant Florencia Escandor Enguerra (NA00015847) with unprofessional conduct and immediately suspended her credential. The state Department of Social and Health Services determined that Enguerra neglected a vulnerable adult. That agency placed her on its long-term care abuse and neglect registry. She can’t be employed in caring for or having unsupervised access to vulnerable adults.
In October 2019 the secretary of health suspended for at least two years the certified nursing assistant credential of Verna Sales Manzano (NC60261025). Manzano took money from the wallet of a visitor at a facility where she worked.
Lewis County
In October 2019 the secretary of health suspended for at least five years the registered nursing assistant credential of Brittanie Pitcher (NA60654977), who in 2019 was convicted of three counts of obtaining a controlled substance by fraud or forged prescription.
Pierce County
In October 2019 the Nursing Assistant Program charged certified nursing assistant Becaley Yolanda Elsie (NC10049409) with unprofessional conduct and immediately suspended her credential. The state Department of Social and Health Services, which determined that Elsie abused a vulnerable adult, placed her on its long-term care abuse and neglect registry. That bars her from being employed in caring for or having unsupervised access to vulnerable adults.
Skagit County
In October 2019 the secretary of health revoked the certified nursing assistant credential of Willie Orlando Lawton (NC10035070). Lawton kissed and had sexual contact with vulnerable adult patients at a rehabilitation center where he worked. In 2019 Lawton was charged with second-degree rape - frail elder or vulnerable adult, indecent liberties - frail elder or vulnerable adult, and two counts of fourth-degree assault with sexual motivation.
Snohomish County
In October 2019 the Nursing Commission suspended for at least 12 months the registered nurse credential of Kristin Leigh Peralta (RN00101340). In 2018 Peralta was charged with driving under the influence, fourth-degree assault-domestic violence, and telephone harassment, domestic violence, phone threat to kill.
Spokane County
In October 2019 the Nursing Assistant Program charged certified nursing assistant Jamie M. Salois, also known as Jamie M. Forrest (NC10060263), with unprofessional conduct and immediately suspended her credential. Upon determining that Salois financially exploited a vulnerable adult, the state Department of Social and Health Services placed her on its long-term care abuse and neglect registry. That prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.
In October 2019 the Home Care Aide Program suspended for at least five years the certified home care aide credential of Lakeychtia Rae-Lynn Rose Garland (HM60887289), who stole items from three patients.
In October 2019 the secretary of health suspended for at least two years the registered nursing assistant credential of Mark Allen Gonzalez (NA60403008). The state Department of Social and Health Services determined that Gonzalez abused a vulnerable adult, and placed him on its long-term care abuse and neglect registry. That means he can’t be employed in caring for or having unsupervised access to vulnerable adults.
Thurston County
In September 2019 the Marriage and Family Therapist Program entered an agreement with marriage and family therapist Michael Dwayne Roos (LF60497884) that indefinitely suspends his credential. Roos didn’t comply with a 2018 stipulation requiring him to submit performance evaluations or unemployment declarations, and to complete continuing education in boundaries.
Yakima County
In October 2019 the Nursing Assistant Program charged certified nursing assistant Mary D. Juarez (NC60391099) with unprofessional conduct and immediately suspended her credential. The state Department of Social and Health Services determined that Juarez financially exploited a vulnerable adult. Her placement on its long-term care abuse and neglect registry prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.
Out of State
Oregon: In October 2019 the Nursing Commission charged registered nurse James Michael Rohde (RN60139934) with unprofessional conduct and immediately suspended his credential. In 2018 Rohde’s Oregon nurse license was revoked in connection with inappropriate sexual contact with a client at work.
Oregon: In October 2019 the Nursing Commission charged registered nurse Joy Sharyn Squires-Jensen, also known as Joy Snow-Workman and as Joy Blum, (RN60376647) with unprofessional conduct and immediately suspended her credential. In 2018 Squires-Jensen’s application to reinstate her Oregon nurse license was denied in connection with failure to fully cooperate with an investigation, failure to provide requested documents, and violating a previous surrender order.
