State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In June 2016 the Nursing Assistant Program entered an agreement with certified nursing assistant Heather A. Hernandez (NC10059549) that suspends her credential for at least two years. Hernandez received a $335 money order from a resident of a facility where she worked.
Clark County
In June 2016 the Nursing Commission entered an agreement with registered nurse Laura A. Flanagan (RN00157933) under which she permanently surrendered her credential. In 2016 Flanagan surrendered her Oregon nurse license and informed the Washington commission she no longer wished to practice because of a medical condition.
King County
In June 2016 the Dental Commission indefinitely suspended the dental assistant credential of Jaime Lynn Clarke (D160231847), who didn’t abide by a substance abuse monitoring contract.
In June 2016 the Massage Program entered an agreement with massage practitioner Yu Fang Cui (MA60293310) that indefinitely suspends her credential. In 2014 Cui relinquished her Florida massage credential in connection with allegations of submitting fraudulent education records.
In June 2016 the secretary of health indefinitely suspended the registered nursing assistant credential of Januario C. Campo (NA60104649). In 2012 Campo was convicted of fourth-degree assault.
In June 2016 the secretary of health indefinitely suspended the radiologic technologist credential of Shelley M. Burns (RT00003832). Burns gave a patient an unknown amount of anesthetic after finishing a body scan; giving the anesthetic was not allowed under her license.
In July 2016 the secretary of health indefinitely suspended the chemical dependency professional credential of Terry William Hake (CP00003479). Hake didn’t adequately respond to a Department of Health investigator’s inquiry about a complaint against him.
In July 2016 the secretary of health suspended the agency-affiliated counselor credential of Jacquelyn Willimon (CG60461796) for at least four years. Willimon had a sexual relationship with a client.
Kitsap County
In June 2016 the secretary of health indefinitely suspended the emergency medical technician credential of Glen R. Rotsten (ES00110815). Rotsten drove recklessly while taking a possible stroke patient for treatment, creating a needless risk of accident and injury, and delaying patient treatment.
Pierce County
In June 2016 the secretary of health indefinitely suspended the certified nursing assistant and X-ray technician credentials of Tracy A. Powell (NC10024365, XT60386642). Powell didn’t comply with a series of substance abuse monitoring agreements.
In June 2016 the secretary of health suspended the radiologic technologist credential of Michael Malley (RT60111099) for at least five years. Before and after he resigned, Malley used another person’s password to take controlled substances from a hospital where he worked.
In June 2016 the secretary of health suspended the certified nursing assistant credential of Jane Ann Piercy (NC60526236) for at least five years. After an incident in which a nursing home client fell and was injured, Piercy tested positive for controlled substances for which she had no prescriptions. She didn’t respond to a Department of Health inquiry.
Skagit County
In June 2016 the secretary of health suspended the massage practitioner credential of Bernadette Kathleen Stanek (MA00011912) for five years. In 2016 Stanek was convicted of four counts of first-degree theft and five counts of forgery.
In June 2016 the Occupational Therapy Board charged occupational therapist Lynne Christine DeBolt (OT60353256) with unprofessional conduct and immediately suspended her credential. DeBolt surrendered her Alaska occupational therapy license in 2014 after not complying with continuing competency requirements.
Snohomish County
In June 2016 the Nursing Commission indefinitely suspended the licensed practical nurse credential of Sherrie Anne Johnson (LP00051705). Johnson’s Arizona nurse license was revoked in 2016 after she didn’t comply with an order to undergo drug testing.
In June 2016 the Nursing Assistant Program entered an agreement with certified nursing assistant Melinda Matta Leonen (NC10006293) that suspends her credential for at least 18 months. While working at an adult family home, Leonen forced a vulnerable adult resident to wear a rubber glove to break up feces in a toilet, and then forced the resident to use her mouth to remove the glove from her right hand.
Spokane County
In June 2016 the Nursing Commission indefinitely suspended the registered nurse credential of Leann C. Noone (RN60020868). Noone removed drugs from an automated system without giving them to patients or reporting that she disposed of them.
In June 2016 the Occupational Therapy Board entered an agreement with occupational therapist Maryellen Boisen (OT00001900) that indefinitely suspends her credential. In 2015 Boisen pleaded guilty to two counts of reckless endangerment. Boisen failed to comply with a substance abuse monitoring program.
In June 2016 the secretary of health indefinitely suspended the certified nursing assistant credential of Rufus Dogba (NC60325098), who threw water on a developmentally delayed resident of a care facility.
Thurston County
In June 2016 the secretary of health suspended the registered nursing assistant credential of William M. Conley (NA60425299) for at least 18 months. Conley yelled at and grabbed a vulnerable resident of a group home.
In June 2016 the secretary of health suspended the registered nursing assistant credential of Teresa Sue Congdon (NA60610103) for at least one year. Congdon falsified patient records and didn’t follow a patient’s treatment plan at a nursing facility where she worked.
Whatcom County
In June 2016 the Pharmacy Commission entered an agreement with pharmacy technician Hazel Devitt (VA00042505) that suspends her credential for at least two years. Devitt diverted narcotics from the pharmacy where she worked.
Yakima County
In June 2016 the Medical Commission charged physician Mark E. Maiocco (MD00034907) with being unable to practice with reasonable skill and safety. The commission and Maiocco entered an agreement that indefinitely suspended his credential. The commission found that Maiocco didn’t comply with a monitoring program.
Out of State
Idaho: In June 2016 the Nursing Commission entered an agreement with registered nurse Linda Ann Swenson (RN00114206) under which she permanently surrendered her credential. Swenson didn’t comply with a substance abuse monitoring contract.
Louisiana: In June 2016 the Nursing Commission indefinitely suspended the registered nurse credential of Erin Lynn Linvog (RN60245004), who didn’t comply with a substance abuse monitoring program.
Oregon: In June 2016 the Nursing Commission indefinitely suspended the registered nurse credential of Danielle Ann Catherine Holcombe-Hoag (RN60335733), who surrendered her Oregon nursing license in 2016.
Texas: In June 2016 the Nursing Commission indefinitely suspended the registered nurse credential of Helen B. Bartholomee (RN00154163). In 2015 her Texas nursing license was placed on inactive status in connection with medication-related issues, and with not notifying staff members that a patient had a reaction to a blood transfusion.
