State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2019 the secretary of health suspended for at least three years the registered nursing assistant credential of Lyndsay Nicole Lapierre (NA60497852). In 2018 Lapierre was convicted of second-degree theft. She stole a client’s electronic benefit transfer card and used it to make purchases for her own benefit.

Clark County

In November 2019 the secretary of health suspended for at least five years the certified nursing assistant credential of Keri Kaylene Picone (NC10036961). The state Department of Social and Health Services, which found that Picone financially exploited a vulnerable adult, placed her on its long-term care abuse and neglect registry. That prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.

Grant County

In December 2019 the secretary of health suspended for at least five years the registered nursing assistant credential of DeAnna R. Castro (NA60656694). In 2019 Castro was convicted of first-degree malicious mischief - physical damage, and of possession of a controlled substance.

King County

In November 2019 the Pharmacy Commission indefinitely suspended the pharmacy technician credential of Coryn L. Schober (VA60285246), who diverted controlled substances from a pharmacy where she worked, and who didn’t respond to a commission investigator’s inquiry.

In November 2019 the Agency-Affiliated Counselor and Certified Counselor programs indefinitely suspended the agency-affiliated counselor and certified counselor credentials of Edward Anthony Desano III (CG60126673, CL60136157), who declined to enter a substance abuse monitoring program.

In November 2019 the Nursing Commission entered an agreement with registered nurse Katerina Marie Falkenberg (RN00127977) that suspends her credential. Falkenberg didn’t complete a required refresher course.

In November 2019 the secretary of health permanently revoked the certified nursing assistant credential of Nshimiyimana Olivier Hamzat (NC60434803). In 2019 Hamzat was charged with two counts of second-degree rape, and one count of indecent liberties, in connection with sexual assault on a patient at a skilled nursing facility where Hamzat worked.

Pierce County

In November 2019 the Department of Health rescinded the certified nursing assistant credential of Sean P. Filomeo (NC60685471), who didn’t pass a required examination.

In November 2019 the Massage Therapist Program entered an agreement with massage therapist Jing Han (MA60635357) that suspends her credential for at least one year. In 2018 Han was convicted of unlawful practice of a professions/operation of a business.

In December 2019 the Nursing Commission charged licensed practical nurse Suleta Marie Kamau (LP60645110) with unprofessional conduct and immediately suspended her credential. Kamau’s Ohio nurse license was suspended in 2018.

In December 2019 the Pharmacy Commission indefinitely suspended the pharmacist credential of Marina L. Hradil (PH00049495), who diverted legend drugs from a pharmacy where she had worked.

Skagit County

In November 2019 the Nursing Assistant Program entered an agreement with certified nursing assistant Monserrath Nohelly Carrisoza-Rojas (NC60784010) that suspends her credential for at least five years. Carrisoza-Rojas used a client’s debit card multiple times to withdraw money and to make purchases for her own use. In 2018 Carrisoza-Rojas was convicted of second-degree attempted theft, and of three counts of third-degree theft.

Snohomish County

In November 2019 the Nursing Assistant Program entered an agreement with certified nursing assistant Pammela J. Curtis (NC10074344) that permanently revokes her credential. A nursing assistant credential issued under Curtis’s name was illegitimately applied for with a false identity, and was used for more than 13 years.

Spokane County

In November 2019 the Substance Use Disorder Professional Program indefinitely suspended the substance use disorder professional trainee credential of Johnathan Daniel D’Auteuil (CO60841282), who didn’t comply with a substance abuse monitoring contract.

In November 2019 the secretary of health suspended for at least 10 years the registered nursing assistant credential of Quanlontina Woodard (NA00199781), who in 2019 was convicted of third-degree assault of a child, and of second-degree assault.

In November 2019 the secretary of health revoked the home care aide credential of Dazee Marie McDowell (HM60862659). In 2019 the state Department of Social and Health Services disqualified McDowell from caring for vulnerable adults, juveniles, and children. That prohibits her from home care aide certification.

Yakima County

In December 2019 the secretary of health suspended for at least five years the certified nursing assistant credential of Mary D. Juarez (NC60391099). The state Department of Social and Health Services determined that Juarez financially exploited a vulnerable adult. Her placement on its long-term care abuse and neglect registry prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.

Out of State

Arizona: In November 2019 the Massage Therapist Program entered an agreement with massage therapist Michael Ross Hanigan (MA60335534) that suspends his credential for at least five years. Hanigan acknowledged having sexual contact with a client, and sending the client inappropriate material.

Colorado: In November 2019 the Nursing Commission suspended the registered nurse and advanced registered nurse practitioner credentials of Mallory Moss Katz (RN60939306, AP60939307) for at least 36 months. In 2019 Katz relinquished her Colorado nursing credentials in connection with having formed a romantic, sexual, and financial relationship with a patient.

Illinois: In November 2019 the Nursing Commission charged registered nurse Sean Robert Hammer (RN60866245) with unprofessional conduct and immediately suspended his credential. Hammer’s Illinois nurse license was suspended in 2019.

Indiana: In November 2019 the Nursing Commission charged licensed practical nurse Mandy Cox (LP60892641) with unprofessional conduct and immediately suspended her credential. Cox’s Louisiana nurse license was suspended in 2019.

New York: In November 2019 the Nursing Commission charged registered nurse Maureen Ann Herlan (RN60621325) with unprofessional conduct and immediately suspended her credential. Herlan’s Oklahoma nurse license was suspended in 2018.

Pennsylvania: In November 2019 the Nursing Commission entered an agreement with registered nurse Timothy P. Kelly (RN60867840) that indefinitely suspends his credential. Kelly’s Pennsylvania nurse license was suspended in 2019 in connection with allegations and criminal charges related to patient abuse.
