The Community Health Assessment (CHA) highlights key factors affecting the health of a community and identifies resources in the state or the community to address these factors. Community leaders, public health agencies, businesses, hospitals, private practitioners, and academic centers, form a community assessment team. Data for the assessment is collected systematically and carefully analyzed. The completed assessment report is shared to disseminate information about the community's health and the community's capacity to improve the lives of residents. The CHA provides a strong basis for promoting health and improving the health of community members.
