Vital Records Closed April 16-17
Vital Records will be closed April 16-17, 2026, during a required relocation of our staff office in the DOH Tumwater Campus. Our public counter in Town Center 2 will be closed, our phone line will be down, and our email inbox will not be monitored. You will not be able to make any appointments for amendment services on April 16-17. Expect slight delays for other Vital Records requests because of this planned closure. Our services will return to normal on Monday, April 20. We apologize for the inconvenience and thank you for your patience.
You can make corrections and changes to birth, death, marriage, and divorce certificates by mail or in person.
In-person services are by appointment only - no walk-ins. Appointments are limited to the following services:
- Corrections
- Parentage
- Adoptions
- Court Ordered Legal Name Changes
Once you submit the appointment request, you will receive an email confirmation with the date, time, and location.
A member of our team may contact you for additional information before the appointment.
Before Scheduling
The Department of Health cannot guarantee same day service during your appointment.
To avoid appointment cancellation or rescheduling, make sure you have all required documentation, including proof documents before going to your appointment.
If you order a certificate during your appointment, there is an additional $13.50 processing fee.
