Grant Application for Community and Faith-Based Organizations

If you are a community or faith-based organization in Washington, we invite you to complete the application for a mini grant to promote awareness and education about brain health and dementia. The deadline to submit the application is 11:59 p.m. on January 16, 2026.

Application Scoring

We hope to provide awards to as many applicants as possible who meet the requirements. However, if we receive more applications than we can fund, a grant advisory group consisting of community leaders and those who serve people in communities most affected by dementia will assist in making the final selections.

Special consideration will be given to applicants serving communities identified with a higher prevalence and risks of dementia based on data from Alzheimer’s Association reports, and to applicants that can describe their ability to fulfill the grant requirements.

The committee will attempt to select a mix of awardees from different types of organizations, locations served, and communities served. Our goal is to bring brain health information and dementia awareness to new areas and to the communities of highest need across the state. The Washington State Department of Health (DOH) will make a notification of award by January 30, 2026.

Application Information Needed

The electronic application form will ask for the following information:

  • Name of person submitting application.
  • Email.
  • Phone.
  • Organization.
  • Organization address (organizations must be physically located in Washington state).
  • Organization website.
  • Choose the term that best describes your organization type: community-based, faith-based, health care, or other.
  • Identify the county or counties where you will hold your 2 educational sessions.
  • Describe your organization's mission and how this work fits into your mission. Limit your response to 600 characters, or approximately 100 words.
  • Describe the community your organization would serve with this grant. Answers may include demographics such as urban/rural and counties/cities served. Answers may also include a focus on a particular ethnic, race, gender, or socioeconomic group in your community. Limit your response to 600 characters, or approximately 100 words.
  • Provide a short summary (600 characters, or about 100 words) of how you plan to meet the required grant activities of:
    • Choosing trusted messengers.
    • Recruiting for and hosting 2 Brain Health and Dementia Awareness sessions for your community, and if those will be in-person or virtual events.
    • Sharing additional information about brain health and dementia with your community.
  • Does your organization plan to choose one of the optional activities to complete? If yes, note the activity you would choose.
  • Describe any previous efforts your agency has taken to address brain health and dementia in your community, how you would build upon that or create new efforts, and what you hope to accomplish if you receive this grant. Limit your response to 600 characters, or about 100 words.
