Complete Annual Reporting

SCCI Wizard

The School and Child Care Immunization Wizard is a tool for questions about immunization requirements, forms, the School and Child Care Immunization Module, roster uploads and delegation requests.

This page is for school, district and child care staff responsible for submitting the annual Immunization Status Report to the Washington State Department of Health. It explains how to use the School and Child Care Immunization Manual (SCCIM) to prepare for and complete the reporting process.

This page outlines the steps required to gather, verify, and submit your school’s immunization compliance data each fall using SCCIM. It includes guidance on when reports are due, how to generate the required data, and what forms are needed.

Annual immunization reporting is required for all Washington schools. Submitting this report ensures statewide compliance with public health policy and helps the Department of Health identify trends in immunization coverage. SCCIM streamlines the reporting process by automatically calculating compliance status and generating summary data.

Using SCCIM for reporting:

  • Eliminates the need to fill out the online survey.
  • Improves accuracy.
  • Standardizes compliance measures.

Prepare Your Data

By December 1, the date the data is pulled from SCCIM tables:

  • Make sure your SCCIM rosters are up to date and accurate for grades and schools. Remove withdrawn students and add new students.
  • Add all vaccination dates, exemptions, and immunity records you have parent/guardian permission to enter.
