Out-of-State Verification Requests

We Offer 2 Methods of Credential Verification:

  • Written verifications - Sent by the Department of Health (DOH), and  
  • Online verifications - Available through the DOH website.

Please note: DOH does not complete forms from other states; we provide our own.

How to Request an Out-of-State Verification:

  1. Log in to your HELMS Portal via Secure Access Washington (SAW).

  2. Navigate to the "All Credentials" tab to view your credentials.
  3. Click on the three dots under "Actions", then select "Request Verification Letter"
  4. Choose a Delivery Method:
    • Email - Enter the recipient's name and email address.
    • Mail - Enter the recipient's name and mailing address.
  5. Click "Submit" and follow the prompts to complete the request, including any applicable fees.

Payments can be made over the phone at 360-236-4700 once you have received your deficiency letter by email or mail. You may also mail in your payment to P.O. BOX 1099, Olympia, WA 98504-7877.

If you made your request online, you can sign back into SAW and pay online. Please do not make duplicate requests to pay. Contact 360-236-4700 for instructions.

Verifications are processed in date order by date received, whether it be by mail, email, or online.  Please contact 360-236-4693 with general verification questions.
