ESSB 5179 (PDF) (Laws of 2023) requires hospice agencies to submit to the department the following:
- Hospice policies regarding end-of-life care
- Hospice Death with Dignity Act form
The purpose of this form is to provide the public with specific information about which end-of-life services are and are not generally available at each hospice agency. Instructions for submitting the form:
