See Survey results (PDF). The most widely accepted insurance codes for the PFAS serum test are billing code CPT 82542 and diagnosis codes ICD-10 Z77.11 “Contact with and (suspected) exposure to environmental pollution” or ICD-10 Z77.111 “Contact with and (suspected) exposure to water pollution”.
