Adverse Events Reporting Data Dashboard

Quarterly Reports

We create adverse events reports every quarter or every three months. These reports represent aggregate or total information, and we post them within 90 days after the reporting quarter. 

We don't share specific details, such as patient and staff identification or the outcome of events, in these reports. If they choose, facilities can provide information on the contextual information form when they report an event. Links to these completed forms are listed below the dashboard.

The adverse events dashboard includes event data from 2014 to the present. We do have data on events dating back to 2006, but this information is not available on the dashboard. If you would like to request information about events that occurred before 2014, email the adverse events program for assistance.

Root cause analysis reports are confidential. We will not release these to the public.
