Adverse Events Reporting Program Contact Information

For questions about reporting requirements, adverse event definitions, check-in survey, quarterly reports or other electronic or online tools, or for other general program inquiries and adverse event notification submissions and updates to facility contact information.

Our reporting system is now secure. You can upload root cause analysis reports through a secure link that is unique for each facility and event. These links are not interchangeable. While uploading is preferred, you can also mail your report if needed.
