Free Vital Record Replacements for Those Impacted by Wildfire
The Department of Health is temporarily providing certificates at no charge in relation to Governor Ferguson's Emergency Proclamation 26-03. Residents who qualify for free services can request replacement WA certificates by emailing contactchs@doh.wa.gov or calling 360-236-4300 (option 8 for Spanish) for assistance. An online platform for requests will be available on Monday, August 24, 2026.
Official Washington State Certificates
The Department of Health issues certified copies of vital records (or “certificates”) for births, deaths, stillbirths, fetal deaths, marriages, and divorces that took place in the state of Washington.
Ordering Vital Records (certificates)
To check the status of your certificate order, please email contactchs@doh.wa.gov or call 360-236-4300.

Important note: VitalChek is our ONLY contracted and approved third-party vendor. Other third-party companies charge customers high fees to “process” or “help apply” for vital record certificates. If you ordered through one of these companies and are unsatisfied, you can file a consumer complaint online at Office of the Attorney General or call 206-464-6684, Monday-Friday, 10:00 a.m. to 3:00 p.m.
