Beginning March 15, 2022, you can make corrections and changes to birth, death, marriage, and divorce certificates by mail or in person.
In-person services are by appointment only and limited to the following services:
- Court Ordered Legal Name Changes
Once you submit the appointment request, you will receive an email confirmation with the date, time, and location.
A member of our team may contact you for additional information before the appointment.
The Department of Health cannot guarantee same day service during your appointment.
To avoid appointment cancellation or rescheduling, make sure you have all required documentation, including proof documents before going to your appointment.
If you order a certificate during your appointment, there is an additional $13.50 processing fee.
