About the Pre-Exposure Prophylaxis Drug Assistance Program (PrEP DAP)
The Pre-Exposure Prophylaxis Drug Assistance Program (PrEP DAP) is offered through the Washington State Department of Health (DOH). It helps people in Washington, who are prescribed PrEP and need help paying for generic PrEP medication, PrEP doctor visits, and PrEP labs.
To get into the program, you must:
- Live in Washington State.
- Test negative for HIV.
- Have gross monthly income at or below 500% of the Federal Poverty Level (FPL). The gross monthly income amount is $6,650 for 2026. Find the 2026 FPL chart here.
- You must send proof of all reported gross monthly income. This is how much you make before any taxes or deductions. Contact PrEPDAP@doh.wa.gov if you are not sure what to send.
- These are the documents we will accept. Send the ones that apply to you:
- If you are employed
- By an employer: Pay stubs from your two (2) most recent months
- Self-employed: A profit and loss statement for your two (2) most recent months (only include deductions the IRS allows, even if you do not file taxes)
- If you receive income from other sources
- Current year award letters for Social Security, Unemployment, or Pension Benefits, etc.
- If you receive cash income
- A profit and loss statement from your two (2) most recent months
- If you are employed
- If you do not have any income, you will explain how you support yourself. We may need to contact you for more information.
PrEP DAP Has an Income Requirement
PrEP DAP is for people who need help paying bills for PrEP medication, PrEP doctor visits, and PrEP tests and have gross monthly income at or below 500% of the Federal Poverty Level (FPL)
PrEP and Health Insurance
PrEP medication, PrEP doctor visits and PrEP tests should be provided at $0 cost. See FAQ for information about what to do about bills for PrEP medications, PrEP doctor visits, and PrEP tests.
The Application Must be Complete
Make sure all sections are complete and is easy to read by typing it or using a blue or black ink pen. You need to provide all required documents. If you leave something out, we will contact you with instructions for sending the missing information. This makes a longer wait time for you.

How to Submit an Application
Once the application is filled out, signed, and dated, choose the way that works best for you:
Option 1: Encrypted Email (Fastest)
This is the quickest way to get us your info. Since your privacy matters, we use encrypted email to keep your data safe.
- How to do it: Send a quick email to PrEPDAP@doh.wa.gov saying you’re ready to submit.
- What happens next: We will send you a secure, encrypted reply. You can then attach your application and income documents directly to that email.
Option 2: Confidential Fax
If you have access to a fax machine, follow these steps to keep your information private:
- Test it: Send a test fax with a cover sheet addressed to "PrEP DAP Applications."
- Check in: Email us at PrEPDAP@doh.wa.gov to make sure we got the test.
- Send it: Once we confirm, you’re good to fax your full application and documents.
Option 3: Mail
You can always send it in the mail. Just print and complete the application, sign and date it, and mail it in with paper copies of all required documents.
Application Checklist:
❑ Proof of Legal Name: Please provide us with a copy of one of the following to verify your full legal name:
- Any State driver’s license or Identification card (can be expired)
- Passport
❑ Proof of WA Residency: If you have a home address, please provide us a copy of one of the following to verify your WA
Residency: Contact us if you are not sure what to send.
- Current Washington State driver’s license or Identification card
- Washington voter registration card
- Utility bill (cell phone bills not accepted)
- Lease/rental/mortgage agreement
❑ Income: If you have income, please provide the required documentation listed on page 2 for all income types received by each person.
❑ Insurance Card: If you have insurance, please provide us a copy of your insurance card.
❑ Application completed in ink or typed
❑ Application filled out completely (Section 1 thru Section 11) with all required documentation, dates and signatures.
