Provider Enrollment

Each year, a completed provider agreement is required prior to the receipt of publicly supplied vaccine.

AVP enrollment and re-enrollment for the 2025-2026 year will begin on June 1, 2026.

If you are a provider who was enrolled in AVP last year who wishes to re-enroll:

  • Your facility contacts (primary/ back-up vaccine coordinators and signatory on file) will receive an automated email on June 1, 2026, with a link to your pre-populated, facility-specific provider agreement to update and sign. Do not use the link below to submit an enrollment inquiry if you are re-enrolling, as this will result in delays.

If you are a provider who was not enrolled in AVP last year:

  • You may use the link below starting on June 1, 2026 to begin enrolling in AVP. Please do not start enrolling prior to June 1 or you will need to complete the enrollment form again.

Annual Vaccine Coordinator Training Requirements

This is an immunization training course created by the Washington State Childhood and Adult Vaccine Programs. Primary and back-up vaccine coordinators must complete this annual training requirement within the current calendar year, prior to their provider agreement renewal deadline. It is also required for all primary and back-up vaccine coordinators that are new to the program.

For dually enrolled AVP/ CVP providers:

2026 Dually Enrolled CVP/AVP Vaccine Coordinator Training

  • Please complete Module 1 first; the rest of the modules can be taken in any order. Please note Module 10 is only required to obtain continuing educations credits. Training modules may be paused and continued later. Remember to print a final certificate for your CVP/AVP Binder and download a copy to upload to Redcap, which is required for AVP renewal.
