This page is for school and child care staff who need to understand and use the Certificate of Immunization Status (CIS) when working with student immunization records. The CIS forms are central to work done in the School and Child Care Immunization Module (SCCIM), whether you're reviewing compliance, updating a student's record, or preparing for reporting.
This page provides an overview of the CIS, explains how it is used within SCCIM, and identifies when a printed CIS is required. The end of the page also includes more resources for understanding and using the CIS.
The CIS is the official immunization form used by all Washington schools and child care centers. SCCIM automatically generates validated CIS forms based on data in the Washington State Immunization Information System (WAIIS). This form shows if a student is compliant with state immunization requirements, and is often required for enrollment, auditing, and communication with families.
Using the CIS correctly reduces manual errors, ensures you’re aligned with state requirements, and helps families clearly understand their child’s immunization status.
Step 1: Understand What the CIS Includes
The validated CIS is designed to measure if a child has met the requirement for full immunity to the diseases required. When printed directly from the WAIIS it includes:
- Vaccines received (and whether they meet state requirements).
- Forecasted doses for future compliance.
- Immunity entered by health care providers marked permanent in special consideration on the vaccination view/add page.
- The compliance status based on age of a child in child care and grade level of a student in school.
The validated CIS does not include:
- Immunity entered in the SCCIM demographic page.
- Exemptions entered on the SCCIM demographic page or by providers on the vaccination view/add page.
- Note: exemptions entered by providers on the vaccination view/add page are not school or child care exemptions. They are documentation of refusal of a vaccination offered at a clinic visit.

Step 2: Generate the CIS in SCCIM
There are three places to generate a CIS in SCCIM:
- Demographic page
- Vaccination View/Add page
- School reports
