The School and Child Care Immunization Wizard is a tool for questions about immunization requirements, forms, the School and Child Care Immunization Module, roster uploads and delegation requests.

This page is for school nurses and administrative staff with access to the School and Child Care Immunization Module (SCCIM) who need to enter or update student immunization exemptions or provider-documented immunity. This task supports accurate reporting and compliance tracking.

This page explains how to enter valid Certificate of Exemption (COE) information and documented immunity into SCCIM. These entries affect a student's compliance status and SCCIM-generated reports.

Both exemptions and immunity must be documented in SCCIM to ensure that your school or child care facility’s reports accurately reflect each student’s immunization status. These records impact compliance calculations and are required for accurate school and district-level reporting.

  • Exemptions entered into SCCIM affect compliance reports, but do not affect the WAIIS forecast or the validated Certificate of Immunization Status (CIS).
  • Immunity entered in SCCIM affect compliance reports but does not impact the WAIIS forecast or the CIS. Only immunity entered by a healthcare provider impacts the CIS and forecast.
  • The WA Immunization Information System forecast, and validated CIS are separate from SCCIM.

To enter an exemption, you must have a completed Certificate of Exemption (COE) form.

To enter immunity, you must have documentation from a healthcare provider stating the student is immune.

  • Acceptable forms include a signed CIS, a provider-signed lab report, or a letter of immunity.
  • A copy of the lab report is no longer required as long as the provider confirms immunity.
  • If your school is subject to FERPA, you must have written parent or guardian consent before entering any exemption or immunity data into SCCIM. For more information, see Privacy, Consent, and Documentation Requirements.
  • Immunity can be documented by antibody titer for:
    • Diphtheria
    • Tetanus
    • Hepatitis B
    • Hib
    • Varicella
    • Measles
    • Mumps
    • Rubella
    • Polio
      • Polio antibody titer is- only allowed if documentation shows immunity to all three polioviruses.
      • Antibody titers to type 2 poliovirus have not been available since 2016, so documenting new immunity to polio by titer is no longer an option.
    • There is no acceptable titer for pertussis and pneumococcal disease.
