  • To establish a 26-station dialysis facility, within Clark.
  • To add five stations to DaVita East Wenatchee Dialysis Center, within Douglas.
  • To establish an eight-station dialysis facility, within Grant.
  • To establish a five-station dialysis facility, within Grays Harbor.
  • To establish a four-station dialysis facility, within King eight.
  • To relocate and add seven stations to Graham Dialysis Center, within Pierce two.

Northwest Kidney Centers (PDF) – multiple proposals, all which expire June 3, 2019.

  • To establish a new 14-station dialysis facility, within Snohomish 2.
  • To establish a new 15-station dialysis facility, within King 3.
  • To establish a new 40-station dialysis facility, by relocating 12 stations from NKC Auburn and adding 28 new stations, within King 11.

  • To relocate four stations from OPKC Coordinated Care Unit to OPKC Bremerton, both within Kitsap.

  • The Polyclinic, PC (PDF) – proposing to operate The Polyclinic Endoscopy Surgery Center as a four-operating room, free standing ambulatory surgical center serving the Central King County planning area. Expires March 25, 2019.
  • Virginia Mason Medical Center (PDF) – proposing to establish a five-bed Level II neonatal service, using existing licensed beds. The service area is King and South Snohomish counties. Expires March 18, 2019.
  • The Alliance for South Sound Health (PDF) – proposing to amend several conditions (#9, #10, and #11) to their existing Certificate of Need #1563, approving them to establish a 120-bed psychiatric hospital in Tacoma, within Pierce County. Expires March 5, 2019.

  • Pacific Cataract and Laser Institute, Inc., P.C. (PDF) – proposing to operate a three-operating room, free-standing ASC that serves the Clark County's secondary planning area. Expires February 18, 2019.
  • Meridian Surgery Center (PDF) – proposes to convert their existing exempt ambulatory surgery center to a Certificate of Need-approved, two-room facility, located in the East Pierce secondary health services planning area. Expires February 13, 2019.
  • Meridian Surgery Center (PDF) – proposes to convert their existing exempt ambulatory surgery center to a Certificate of Need-approved facility, located in the East Pierce secondary health services planning area. Expires February 13, 2019.
  • Swedish Health Services (PDF) – proposes to convert six existing acute care beds into a six bed pediatric intensive care unit on the Swedish Issaquah campus, in the East King Secondary Health Services planning area. Expires February 8, 2019.
