The Death with Dignity Act requires the medical certifier to only list the underlying terminal condition as the cause of death on the patient’s death certificate. The death certificate must not mention or indicate a patient participated in Death with Dignity.
Use these guidance documents to help you complete the cause of death section of the death certificate and ensure compliance with requirements of the Washington Death with Dignity Act:
- Instructions for Qualified Medical Providers (DOH 422-151)
- Instructions for Funeral Directors (DOH 422-147)
- Instructions for Local Registrars (DOH 422-150)
- Instructions for Medical Examiners and Coroners (DOH 422-148)
