OLYMPIA – The Washington State Department of Health has issued fines and updates to enforcement actions on the following health care facilities in our state.
The Department of Health protects and promotes public health, safety, and welfare in Washington by regulating the competency and quality of health care providers and facilities. The agency establishes, monitors, and enforces qualifications for licensing, consistent standards of practice, continuing competency mechanisms, and discipline. Rules, policies, and procedures promote the delivery of quality health care to people in Washington.

King County
The Department of Health has issued a notice of intent to issue civil fines on the license of St. Anne Hospital, formerly Highline Medical Center (HAC.FS.00000126).
Thurston County
The Department of Health has issued a notice of intent to issue a civil fine on the license of St. Peter Hospital (HAC.FS.00000159).
The notice alleges that St. Peter Hospital, located at 413 Lilly Road NE, Olympia, WA 98506-5133, failed to correct deficiencies previously cited during an investigation regarding a patient safety concern. The hospital was issued a civil fine under WAC 246-320-013 in the amount of $2,000 for a repeat deficiency found. The violation is related to the hospital not completing monthly morbidity and mortality reviews of all patient deaths as required. The hospital has corrected the issue. They have 28 days to request a hearing or pay the fine. The public will continue to be updated.
The Department of Health (DOH) has issued a final order suspending the license of Integrity Care and Staffing Inc. (IHS.FS.61014907).
The statement of charges issued in July 2024 alleged that Integrity Care and Staffing Inc. (Integrity Care), located at 510 Grass Lake St. NW, Olympia, WA 98502, failed to cooperate with a complaint investigation regarding concerns that the facility had been accepting payment for a Puyallup Tribe client since November of 2023, but had failed to provide patient care services. Integrity Care denied access to DOH investigators over the course of multiple attempts and several months. Integrity Care requested a hearing to contest the charges, and a Health Law Judge entered an initial order in April 2025 suspending Integrity Care’s license for a period of five years amongst other stipulations. Integrity Care did not respond to the initial order and their license is now suspended. Integrity Care is not allowed to operate as a licensed in-home services facility in the state of Washington.
