OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
King County
In April 2025 the Dental Quality Assurance Commission withdrew amended 2024 charges against dentist Calley Justin Christie (DE60470632), who also holds a general anesthesia permit (GA60614575).
In April 2025 the Department of Health granted Edward Mwangi Macharia’s (CG61588160, NC61583262) agency affiliated counselor and certified nursing assistant credentials with conditions on the licenses.
In April 2025 the Dental Quality Assurance Commission released Andrew Young (DE60114186) from the terms and conditions of a 2022 agreed order.
Pierce County
In April 2025 the Department of Health terminated the probation on Theresa Gwen Banzet’s (CO61360518) substance use disorder professional credential.
In March 2025 the Department of Health denied Carissa A. Prunty’s (HM61580081) application for a home care aide credential. The denial was based on findings of unprofessional conduct, including inappropriate communication with a former client and admitting to purchasing alcohol for a client while employed as a home care aide.
In March 2025 the Department of Health charged certified nursing assistant Hony I. Wilkinson (NC10047121) with unprofessional conduct. In November 2024, the Department of Social and Health Services determined Wilkinson neglected a vulnerable adult and placed Wilkinson on its long-term care abuse and neglect registry. Wilkinson did not appeal the finding.
Spokane County
In April 2025 the Department of Health granted Phylicia Carpio Adams (CO61662196) a substance use disorder professional trainee credential, placing it on probation for two years.
In April 2025 the Department of Health charged registered nursing assistant Kaylee Kalani Ziel (NA60883900) with unprofessional conduct. In August 2024, Ziel was charged with fourth-degree assault. In October 2024, Ziel agreed to participate in a mental health treatment program in exchange for dismissal of the charge upon successful completion.
Thurston County
In April 2025 the Department of Health charged agency affiliated counselor Cameron Bernice Capps (CG60858238) with unprofessional conduct. Capps allegedly engaged in an inappropriate romantic relationship with a resident while working as a counselor at a juvenile rehabilitation facility.
Whitman County
In April 2025 the Veterinary Board of Governors granted Emma Flieg (VM61663733) a veterinary medication clerk credential with conditions on the license.
In April 2025 the Department of Health charged registered nursing assistant and certified nursing assistant Amy May Foster (NA61286824, NC61515722) with unprofessional conduct. Foster allegedly transported a nursing home resident to her apartment, where they consumed alcohol, and the resident stayed overnight.
Yakima County
In April 2025 the Department of Health terminated the probation on Miguel Angel Bautista’s (CO61191490) substance use disorder professional trainee credential.
In April 2025 the Department of Health charged certified nursing assistant Jenny Estefany Garcia Vilchis (NC60538149) with unprofessional conduct. Garcia Vilchis allegedly took photos of multiple residents without permission and shared them with non-staff outside the facility.
Out of State
Oregon: In May 2025 the Dental Quality Assurance Commission charged dentist Scott Robert Yeaman (DE00010378) with unprofessional conduct. In 2022 and 2023, Yeaman allegedly failed to notify patients of an office closure, did not arrange for continuing care, and did not provide requested dental records.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
