OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In April 2025 the Department of Health denied Devin A. McNeil’s (LH61518407) application for a mental health counselor credential, citing his prior agreement to surrender his credentials as a mental health counselor associate and agency affiliated counselor.
In April 2025 the Department of Health denied Jerilyn Marie Sisich’s (CO61631286) application for a substance use disorder professional trainee credential based on her 2014 and 2017 gross misdemeanor convictions for third-degree theft.
Clark County
In May 2025 the Veterinary Board of Governors terminated the probation on Mark Joel Burton’s (VT00003045) veterinary credential.
In May 2025 the Department of Health charged massage therapist Kawika Maraccini (MA61330620) with unprofessional conduct. In July 2024, Maraccini allegedly engaged in inappropriate sexual contact with a patient during a massage session.
Island County
In May 2025 the Department of Health charged home care aide Brandie Michelle Doss (HM61241763) with unprofessional conduct. Doss allegedly used a patient’s credit card to make unauthorized purchases totaling $165.63.
King County
In April 2025 the Department of Health denied Julio Cesar Tapia Hernandez’s (CM61581852) application for a certified medical assistant credential. The denial was based on May 2013 criminal convictions for first-degree rape of a child and indecent liberties.
In May 2025 the Department of Health charged registered nursing assistant Malaika Wanjiru Karanja (NA61306627) with unprofessional conduct. In February 2024, Karanja allegedly neglected a vulnerable adult in her care and was charged with third-degree criminal mistreatment and third-degree theft. She also failed to respond to an investigator’s request for information.
In May 2025 the Department of Health issued a cease-and-desist order to Iryna Sydorchuk. From 2022 to August 2024, Sydorchuk allegedly advertised and represented herself as a massage therapist on social media without holding a credential to practice massage therapy in Washington.
In April 2025 the Department of Health granted Elliott Woehler’s (MX61656073) application for a probationary mental health counselor credential, placing it on probation until August 2027. During the probationary period, Woehler must practice with oversight from an approved supervisor, submit regular performance or unemployment reports, and comply with all terms of the agreement.
Okanogan County
In May 2025 the Department of Health reinstated Jacki Rae Tasker’s (HM60334877) home care aide credential.
Pierce County
In May 2025 the Department of Health issued a cease-and-desist order to Elizabeth Aleman. Aleman allegedly offered body contouring services in Washington that involved massage and medical procedures without holding a credential to practice as a massage therapist or physician.
In March 2025 the Department of Health denied Valerye June Bolin’s (NC61548843) application for a certified nursing assistant credential based on findings related to the misuse of alcohol and controlled substances, as well as prior criminal convictions for driving while intoxicated.
In May 2025 the Department of Health charged registered nursing assistant and agency affiliated counselor Danny Frank Jablonski (CG60888877, NA61356815) with unprofessional conduct. In March 2024, Jablonski allegedly arrived at work impaired and admitted to drinking alcohol before his shift at a health care facility.
In May 2025 the Department of Health charged registered and certified nursing assistant Daniel Kamau Joseph (NA61505889, NC61569965) with unprofessional conduct. Joseph allegedly accepted multiple loans from a vulnerable adult in his care. An Adult Protective Services investigator also reported video evidence showing Joseph inappropriately touching a patient.
In May 2025 the Department of Health and David Njuguna Ngigi (NC60428371) entered an agreed order reinstating his certified nursing assistant credential and placing it on probation for at least 18 months. As part of the agreed order, Ngigi must submit regular employer performance reports or unemployment declarations, complete continuing education in law and ethics, and comply with evaluation and treatment related to a prior criminal case.
In May 2025 the Department of Health charged associate social worker and agency affiliated counselor Alixaundrea Corrin Poli (CG61117695, SC61098026) with unprofessional conduct. In April 2023, Poli was found unresponsive and subsequently received care related to personal health concerns.
Snohomish County
In January 2025 the Chiropractic Quality Assurance Commission charged chiropractor Devin Allen Brossard (CH60262639) with unprofessional conduct. Between November 2021 and May 2022, investigators observed repeated noncompliance with COVID-19 mask requirements at Brossard’s clinic. He allegedly failed to enforce masking, issued unjustified medical exemptions, and later refused to provide complete patient records to the Commission.
In May 2025 the Department of Health granted Kimiam Waters (CG61635709) a credential to practice as an agency affiliated counselor, placing it on probation for three years. During the probationary period, Waters must work under an approved supervisor, submit regular performance or unemployment reports, and comply with all terms of the agreement.
Spokane County
In April 2025 the Department of Health denied Toniah Brister’s (NA61512448) application for a registered nursing assistant credential. The denial was based on findings of unprofessional conduct, including a prior DUI conviction and failure to complete a required substance use evaluation.
In May 2025 the Department of Health charged massage therapist Katie Michelle Hough (MA61374245) with unprofessional conduct. Hough allegedly advertised and performed large animal massage without the required endorsement, exceeding the scope of her massage therapist credential.
In May 2025 the Department of Health terminated the probation on Krystal Kim Rose’s (CG61350349, CO61132701) substance use disorder professional trainee and agency affiliated counselor credentials.
In May 2025 the Department of Health granted Kayla Ann Tompkins (CO61053913) a credential to practice as a substance use disorder professional trainee, placing it on probation for three years. During that time, Tompkins must practice with oversight from an approved supervisor, submit regular performance or unemployment reports, and comply with all terms of the agreement.
Stevens County
In May 2025 the Pharmacy Quality Assurance Commission charged pharmacist Richard D. Manson (PH00019644) with unprofessional conduct. Manson allegedly failed to comply with a stipulated agreement requiring him to undergo a mental examination.
Thurston County
In May 2025 the Department of Health charged certified nursing assistant April Dawn Irene Hale (NC60500568) with unprofessional conduct. Between August 2019 and June 2023, Hale allegedly failed to maintain professional boundaries while serving as a caregiver in a patient’s home.
Yakima County
In May 2025 the Department of Health withdrew charges against home care aide Denielle Renee Lee (HM60958388).
Out of State
Oregon: In May 2025 the Department of Health charged marriage and family therapist associate Sarah Beaulieu (MG61212557) with unprofessional conduct. In October 2024, the Oregon Board of Licensed and Professional Counselors and Therapists entered an agreed order with Beaulieu requiring her to surrender her license after finding she engaged in an inappropriate relationship with a former patient and solicited more than $6,500 through an online account. The Oregon Board also denied her application to renew her license.
Texas: In May 2025 the Pharmacy Quality Assurance Commission granted Fitz Glasgow (PH61495593) a pharmacist credential, placing it on probation for at least one year. During the probationary period, Glasgow must practice with oversight from an approved supervisor, submit regular employer or unemployment reports, complete continuing education in pharmacy regulations, and comply with all terms of the agreement.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
