State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Benton County

In July 2024 the secretary of health charged registered nursing assistant Tuesdae Arizona Hunter (NA60088578) with unprofessional conduct. In July 2023, Hunter was allegedly seen multiple times by a fellow staff member taking food and household items from the facility she worked at.

Klickitat County

In June 2024 the Dental Quality Assurance Commission denied the dental assistant license application of Lindsey Kaylenn Conn (D161510425). In January 2024, Conn failed to respond to or submit additional requested information to a DOH investigator.

Pierce County

In July 2024 the secretary of health granted Trevonte LaRose (CG61522553) a license to practice as an agency affiliated counselor with conditions on the license. The agency affiliated counselor license is placed on probation for at least two years, during which time LaRose must be supervised by a qualified health care professional who submits quarterly performance evaluation reports, and he must notify his current and future health care employers of the agreement to practice with conditions. LaRose must also reimburse costs of $500. Between April 2022 to May 2022, LaRose was terminated for recording false client session times in his employer’s time tracking system.

In July 2024 the secretary of health terminated the probation on the registered nursing assistant license of Lucy Njeri Mubeh (NA60419680).

Skagit County

In July 2024 the Veterinary Board of Governors and Lawrence James Campbell (VT00001774) entered an agreed order placing conditions on his veterinarian license. Campbell must pay a $1,000 fine and complete four hours of continuing education in the areas of ethics and medical recordkeeping. In July 2022, Campbell failed to provide a patient’s complete medical records within 10 working days upon receipt of request from another veterinarian.

Snohomish County

In June 2024 the Board of Nursing (formerly the Nursing Commission) denied the registered nurse license of Mercy Akoth Ogola (RN61113597). In March 2024, Ogola pled guilty to and was convicted of fourth-degree assault, a gross misdemeanor, based on allegations that in April 2021, Ogola physically abused a disabled client.

Spokane County

In July 2024 the secretary of health withdrew the statement of charges against certified medical assistant Annabel Morando (CM60362194).

In June 2024 the secretary of health granted the substance use disorder professional license of Brandon John Search (CP61523090) with conditions on the license. The substance use disorder professional license is on probation until at least February 2025, during which time Search must be supervised by a qualified health care professional who submits quarterly performance evaluation reports. Search must also notify his current and future health care employers of the agreement to practice with conditions. In February 2022, Search entered an agreement to practice with conditions with the secretary of health.

In July 2024 the secretary of health charged substance use disorder professional Magen LeeAnn Lloyd (CP60665244) with unprofessional conduct. In January 2022 and July 2022, Lloyd provided clinical services to a patient. This patient was employed and supervised by Lloyd at the facility in March 2023. Between June and August 2023, Lloyd allegedly engaged in a sexual relationship with this patient.

Out of State

Idaho: In June 2024 the secretary of health denied the registered nursing assistant and home care aide licenses of Hailey Anne Boos (NA61396929, HM61440231). In November 2023, Boos failed to respond to an investigator’s request for additional information regarding a client’s allegations against her.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
