OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In May 2022 the Nursing Commission charged registered nurse Daphne Lelora Tobin Jacobsen (RN60560549) with unprofessional conduct for allegedly working as a nurse while impaired by alcohol.
Cowlitz County
In March 2022 the secretary of health denied the registered nursing assistant application of Justine Mia Johnson (NA61235320) because she did not submit a substance use evaluation report required for her application.
Island County
In May 2022 the Nursing Assistant Program charged certified nursing assistant Jack Parker Stewart III (NC60509469) with unprofessional conduct. Stewart allegedly violated a patient’s care plan for transferring the patient from a wheelchair to a bed, which resulted in injury to the patient.
King County
In April 2022 the Nursing Commission charged registered nurse Corina Saldivar (RN60474579) with unprofessional conduct. Saldivar allegedly removed hydromorphone for several patients but failed to document the administration or wastage of the medication.
In April 2022 the Nursing Commission reinstated with conditions the registered nurse license of Nancy Alice O'Leary (RN00110987). O'Leary, who agreed to the conditions, will participate in a substance use monitoring program.
In April 2022 the secretary of health ended conditions on the interim medical assistant, certified medical assistant, and certified nursing assistant credentials of Juiles Yvette Smith (IC60420324, CM60420323, NC10042799).
Kitsap County
In March 2022 the secretary of health granted with conditions the home care aide license of Paul-John Santos Flores (HM60535647). Flores agreed to the conditions, which include an evaluation process and participation in a substance use program.
Pierce County
In March 2022 the secretary of health denied a massage therapist license to Darian Elizabeth Nemec (MA61220241). In 2019, Nemec was convicted of driving under the influence of intoxicants and driving with a suspended or revoked license in Oregon.
In March 2022 the Nursing Commission denied the licensed practical nurse application of Shavon Renee McGuinness (LP60662086). McGuinness used the wrong form and submitted a transcript containing errors and inconsistencies. The educational institution that McGuiness named as having trained her has been implicated in fraudulent transcript schemes and is unauthorized to grant nursing degrees.
In April 2022 the Nursing Assistant Program charged registered nursing assistant Alexandra Smith-Rosas (NA61172518) with unprofessional conduct for allegedly taking controlled substances during work.
Spokane County
In May 2022 the Nursing Assistant Program charged registered nursing assistant Beverly Ann Moore (NA00200586) with unprofessional conduct for allegedly taking medication from a patient for her own use.
Whatcom County
In April 2022 the Nursing Commission reinstated with conditions the registered nurse license of Matthew J. Danz (RN00148293). Danz agreed to the conditions, which include probation until he completes a refresher course and participation in a substance use monitoring program.
Out of State
Oregon: In May 2022 the Nursing Assistant Program charged certified nursing assistant Julia J. Black (NC60393654) with unprofessional conduct. Charges state that Black had a romantic and sexual relationship with a client and accepted money from that client to pay off part of her car.
Texas: In April 2022 the Nursing Commission charged Yangmotso Bhutia Antso (LP60196601) with unprofessional conduct. Antso, whose licensed practical nurse credential is expired, allegedly attempted to obtain a registered nurse degree without acquiring the necessary education. Antso also allegedly misrepresented facts during an investigation by the Nursing Commission.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
