State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Clallam County

In April 2023 the Pharmacy Commission and Alison Louise Dick (VB61250548) agreed to conditions on her pharmacy assistant license. Dick must participate in a substance use monitoring program until the program determines it is no longer necessary.

Clark County

In April 2023 the secretary of health and April J Bassett (CO61393310) agreed to the conditions on her substance use professional trainee license. The conditions require that Bassett’s license is on probation for three years and that she submit quarterly performance reports.

Grays Harbor

In March 2023 the Department of Health notified Kayla Ann Mullin-Glassock (NC61242129) that it had issued her certified nursing assistant license in error and the status was changed to “pending.”

King County

In April 2023 the Substance Use Disorder Professional Program charged substance use disorder professional trainee Denis Edward Jesse LeClech (CO61026236) with unprofessional conduct. Charges state that in August 2021, LeClech is alleged to have tipped off a client to the dates of her upcoming random urine analysis, received payment from her, and then in March 2022 began a sexual relationship with her.

In April 2023 the secretary of health issued a cease-and-desist order to Amin H. Makeen. Makeen is ordered to permanently cease all practice of acupuncture, East Asian medicine, and massage therapy unless he obtains appropriate licensure. Makeen must also pay a $1,000 fine.

In April 2023 the secretary of health ended the probation on the agency affiliated counselor license of Alexander Anthony Coleman (CG61104293).

In April 2023 the Veterinary Board of Governors and Emily Ann Warren (AT61345999) agreed to the conditions on her veterinarian technician license. The conditions are two years of probationary supervision and quarterly performance evaluation reports. Between 2016 and 2018, Warren was convicted of third-degree escape, vehicle prowling, solicitation of a controlled substance with no prescription, and theft.

In March 2023 the Chiropractic Commission modified its agreed order against chiropractor Ronwynn Bruton Pratt (CH00033994). Pratt’s credential will be placed on monitoring for at least 33 months, during which she must contact a substance use monitoring program to determine if she is required to enroll and participate. She must also undergo a mental health evaluation by a mental health professional pre-approved by the Commission and pay a $1,000 fine. In July 2020 Pratt was convicted of fourth-degree assault and domestic violence and third-degree malicious mischief.

Kitsap County

In April 2023 the Dental Commission ended conditions on the dentist license of John J Bial (DE00006310).

Lewis County

In March 2023 the secretary of health ended the conditions on the emergency medical technician license of Marc William Shettlesworth (ES61184872).

Okanogan County

In April 2023 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Toni Sheree Waller (RN00174705, AP61015469-NP) with unprofessional conduct. Charges against Waller allege multiple examples of patient care that fell below the standard of care, including poor documentation, inadequate or inappropriate care, and a lack of evidence-based decision making.

Pacific County

In March 2023 the Nursing Commission amended charges of unprofessional conduct against registered nurse Sandra Kay Donato (RN00116820). Charges allege that Donato failed to comply with a substance use monitoring contract.

Snohomish County

In April 2023 the secretary of health reinstated the certified nursing assistant license of Modou Lamin Dibba (NC60724013).

In April 2023 the Veterinary Board of Governors and veterinarian Robert Jeffrey Mayo (VT00004785) agreed to an order that he attend and complete 16 hours of continuing education in the areas of ethics and record-keeping, remove all advertisements or website statements that he is an animal surgeon, pay a $1,000 fine, and take and pass the Board’s jurisprudence exam. Mayo misrepresented himself as a specialist in veterinary medicine surgery but is not a board-certified surgeon, and in 2019 performed substandard knee surgery on a dog.

In March 2023 the secretary of health ended the conditions on the certified nursing assistant license of Julia Davis Keels (NC60451367).

Spokane County

In April 2023 the secretary of health reinstated the certified nursing assistant and registered nursing assistant licenses of Kayla Marie Lepene (NC60429824, NA60410102).

In April 2023 the Veterinary Board of Governors ended the probation on the veterinary license of Julia Ann Leese (VT00005168).

In April 2023 the Dental Commission charged expanded function dental auxiliary Jennifer Kay Preble (D260139154) with unprofessional conduct. In 2020 Preble agreed to participate in a substance use monitoring program. Charges state that on numerous occasions in 2020, 2021 and 2022, Preble tested positive for alcohol, marijuana, kratom, and prescribed lorazepam.

In April 2023 the secretary of health and Mitchell A Eggett (CO61385524) agreed to conditions on his substance use professional trainee license. The conditions are two years of probationary supervision and quarterly performance evaluation reports. In August and November of 2019, Eggett was convicted of obstructing an officer, vehicle prowling, identity theft and theft.

Thurston County

In April 2023 the Medical Assistant Program charged certified medical assistant-phlebotomist Erin Elizabeth Blair (PC61177084) with unprofessional conduct. Charges state that in September 2021, Blair is alleged to have performed an incorrect ultrasound that was also outside of her scope of practice.

Yakima County

In April 2023 the Nursing Assistant Program charged certified nursing assistant Julie Michelle Reneau (NC60939059) with unprofessional conduct. In May 2022, Reneau is alleged to have admitted to smoking meth.

In April 2023 the Pharmacy Commission charged pharmacist David Alan Hicks (PH00045764) with unprofessional conduct. In 2019, Hicks dispensed the wrong prescription to a patient, who ingested it and underwent urgent care. Hicks has since allegedly failed to complete four hours of continuing education in the area of medication errors within the required time period.

In April 2023 the secretary of health withdrew the charges of unprofessional conduct against osteopathic physician and surgeon Larry Sherman Lefors (OP00000767).

Out of State

Oklahoma: In March 2023 the Nursing Commission ended the conditions on the licensed practical nurse credential of Tracy Lynn Hudson (LP60636125).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
