State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Clallam County

In June 2023 the Pharmacy Quality Assurance Commission denied the pharmacy assistant license of Adriana Torres-Janssen (VB61374827). In February 2022, Torres-Janssen was convicted of driving under the influence, a gross misdemeanor.

Franklin County

In July 2023 the secretary of health and Cristian Alexis Guzman (HM61255007) agreed to conditions on his home care aide license. Guzman must contact, enroll, and participate in a substance use monitoring program. In December 2020, Guzman was convicted of driving under the influence and driving with a suspended license, both third-degree gross misdemeanors. In January 2021, Guzman was convicted of driving under the influence, a gross misdemeanor.

King County

In July 2023 the Chiropractic Quality Assurance Commission charged chiropractor Justin T. Goslin (CH60264834) with unprofessional conduct. Charges state that in 2020, Goslin inappropriately touched a patient during treatment and discussed his personal relationship with the patient. In July 2022, Goslin allegedly smelled of alcohol, discussed his personal relationship with a patient and attempted to keep her from leaving the treatment session and parking lot.

In July 2023 the Board of Nursing (formerly the Nursing Commission) ended the terms and conditions on the registered nurse license and advanced registered nurse practitioner license of Mariya Igor Fair (RN60320185, AP60324589-CRNA).

In July 2023 the Dental Quality Assurance Commission charged dentist Kuzi S. Hsue (DE00006736) with unprofessional conduct. Charges state that Hsue extracted 20 teeth from a patient’s mouth instead of six teeth per the agreement with the patient. Hsue allegedly did not acknowledge the mistake during the procedure when the patient attempted to inform Hsue, nor did Hsue tell the patient’s son and interpreter about the mistake post-op. Hsue allegedly did not have complete or accurate patient records, nor did he provide post-op care instructions to the patient.

In June 2023 the Dental Quality Assurance Commission ended the terms and conditions on the dentist license of Garrett J. Buck (DE60879751).

In July 2023 Unlicensed Practice Program issued Wenyu Li a cease-and-desist order for practicing massage without a license. Li must immediately cease and desist from any and all conduct in the practice of massage therapy. He must also pay a $1,000 fine.

Kitsap County

In July 2023 the Marriage and Family Therapist Program charged marriage and family therapist Timothy James Carver (LF61110001) with unprofessional conduct. Charges state that between July 2021 and February 2022, Carver initiated a sexual relationship with a patient he was providing therapy services to. By March 2022, Carver ended the professional relationship to pursue a sexual relationship with the patient.

Pierce County

In July 2023 the secretary of health ended the conditions on the registered nursing assistant license of Ranicia Camacho (NA60999019).

Snohomish County

In July 2023 the Nursing Assistant Program charged certified nursing assistant Samantha N. Steele (NC10088080) with unprofessional conduct. Charges state that Steele engaged in sexual misconduct with a patient while providing home care assistance in exchange for cash payments. Steele allegedly exchanged personal and sexual messages and engaged in physically intimate or sexual actions with the patient.

In July 2023 the Hypnotherapist Program and Mental Health Counselor program charged hypnotherapist and mental health counselor associate Allison Grace Elmlund (HP60349905, MC60959389) with unprofessional conduct. Charges state that in March 2021, Elmlund provided mental health counseling to a patient even though she was not permitted to provide the service without working under an approved supervisor. Between October 2021 and April 2022, Elmlund allegedly had a therapist-patient relationship and a social relationship with a patient. In June 2022 a subpoena for patient records was sent to Elmlund and she allegedly failed to respond or provide the records.

Spokane County

In July 2023 the Unlicensed Practice Program notified Aimee Denning Hauer (MA00020327) of its intent to issue a cease-and-desist order. In February and March 2023, Hauer allegedly practiced massage therapy with an expired license.

In June 2023 the secretary of health denied the home care aide license of Joshua John Boger (HM61386670). Boger is being investigated for alleged sexual abuse of a vulnerable adult patient.

Thurston County

In May 2023 the secretary of health denied the home care aide application of Jamie Elaine Winchester (HM61107053). Winchester failed to respond to numerous Department of Health attempts to contact her. Between February 2021 and September 2021, Winchester allegedly wrote unauthorized checks and used a client’s debit/credit cards for unauthorized purchasing totaling $37,891.55.

Out of State

Montana: In June 2023 the Board of Nursing Home Administrators reinstated the nursing home administrator license of Warren Curtis Taylor (NH60019876).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
