State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Benton County

In January 2025 the secretary of health charged certified nursing assistant Casandra Maria Chavez (NC60726065) with unprofessional conduct. In April 2024, Chavez allegedly took a patient to purchase marijuana and items from an adult sex store for personal use. Chavez failed to respond to a letter of cooperation from August 2024.

Chelan County

In January 2025 the Pharmacy Quality Assurance Commission denied the pharmacy assistant license of Timothy L. McDaniel (VB61244740). In March 2017, McDaniel was placed on the Department of Social and Health Services long-term care abuse and neglect registry for allowing  CNA medical technicians to administer Lovenox injections to a vulnerable adult.

Cowlitz County

In January 2025 the Dental Quality Assurance Commission released Alesia Marie Richardson (D160056631) from the terms and conditions on her dental assistant license.

Franklin County

In January 2025 the secretary of health granted Cristian Alexis Guzman’s (NC61542908) certified nursing assistant license with conditions. Guzman must continue to comply with a recovery and monitoring program. In December 2020, Guzman was convicted of driving under the influence and third-degree driving while license suspended, both gross misdemeanors. In January 2021, Guzman was convicted of driving under the influence, a gross misdemeanor.

In January 2025 the secretary of health charged certified nursing assistant Chantel Dawn Austin (NC60317140) with unprofessional conduct. Alleged facts state that in November 2023, Austin borrowed and operated a client’s vehicle while impaired by an intoxicating drug. Austin was also arrested for unlawful use of the client’s credit card. In April 2024, Austin pled guilty to felony criminal possession of a financial transaction card, financial transaction number and FTC forgery devices. Austin failed to respond to a letter of cooperation from April 2024.

King County

In January 2025 the secretary of health terminated the probation on the agency affiliated counselor license of Reachny Chea Chau (CG61265044).

In January 2025 the secretary of health terminated the probation on the agency affiliated counselor license of Kiara Monet Patterson (CG61294531).

In January 2025 the secretary of health reinstated Justin T. Goslin’s (CH60264834) license to practice as a chiropractor with conditions. The license is on probation for at least one year, during which time Goslin must allow unannounced audits for up to ten client records.

In January 2025 the Dental Quality Assurance Commission charged dentist Lance B. Timmerman (DE00008229) with unprofessional conduct. From August 2015 to May 2022, Timmerman failed to maintain complete and accurate records for multiple patients’ treatments, consults, visits or billing.

Kitsap County

In January 2025 the secretary of health charged licensed mental health counselor Shawn Michael Adams (LH60177969) with unprofessional conduct. Alleged facts state that in December 2021, Adams was terminated after the wellness center he worked at determined his text message communication with a client was inappropriate. In October 2022, Adams was issued a one-year protection order after repeatedly removing clients from school without permission from their guardians. Between February and August 2022, Adams failed to report at least three disclosures about possible child abuse to proper authorities. Adams also failed to report two instances of child abuse that were disclosed to law enforcement and child welfare authorities. In January 2023, Adams signed an apartment lease for another client and failed to report finding the client’s children unattended at home to the proper authorities.

Lincoln County

In January 2025 the secretary of health granted Catherine Marie Herrman (CO61619529) a substance use disorder professional license with conditions. The license is on probation for at least two years. During this time, Herrman’s supervisor must submit quarterly performance evaluation reports. In September 2021, Herrman pled guilty to theft. In December 2021, Herrman pled guilty to second-degree identity theft and second-degree theft. In July 2022, Herrman pled guilty to first-degree escape.

Pierce County

In January 2025 the secretary of health terminated the probation on the substance use disorder professional trainee license of Norman Lee Jones (CO61287651).

In January 2025 the secretary of health granted Tania Ashanti Rodgers’ (CP61578207) substance use disorder professional license with conditions. Rodgers must continue to comply with her May 2023 agreed order, pass an ethics course, and allow unannounced audits for 10 patient records. Rodgers’ health care supervisor must also submit quarterly performance evaluation reports. In May 2023, an agreed order placed certain conditions on Rodgers’ chemical dependency professional trainee, agency affiliated counselor, and certified nursing assistant licenses.

In January 2025 the secretary of health charged certified nursing assistant Mona L. Hunter (NC10013081) with unprofessional conduct. In August 2023, Hunter allegedly solicited a patient to move into her home and pay $3,000 monthly for care, room and board. Hunter also purchased a vehicle from the patient and became the patient’s power of attorney.

Snohomish County

In January 2025 the Chiropractic Quality Assurance Commission charged chiropractor Devin Allen Brossard (CH60262639) with unprofessional conduct. Alleged facts state that in November 2021, the wellness center owned by Brossard failed to follow the mask mandate and display signs or other notices referencing the mask mandate or other COVID-19 related information. Brossard failed to disclose complete patient medical records as requested in a February 2022 letter of cooperation. In April 2022, Brossard refused again to provide the complete medical records for his patients.

In January 2025 the Board of Hearing and Speech charged hearing aid specialist Yu-Ting Angelina Kong (HA60736242) with unprofessional conduct. From March 2019 to May 2023, Kong allegedly entered e-signatures on clinical documents on behalf of several patients without being designated as their power of attorney. In April 2023, Kong allegedly submitted clinical documentation for a deceased patient.

In January 2025 the secretary of health released Danielle Denise Caldwell (MR60899512) from the terms and conditions on her medical assistant license.

Spokane County

In January 2025 the secretary of health granted Erica Marie Bundy’s (CG61639903, CO61625574) substance use disorder professional trainee and agency affiliated counselor licenses with conditions. The licenses are on probation for at least two years. During this time, Bundy must have her supervisor submit quarterly performance evaluation reports. In July 2021, Bundy was convicted of possession of a controlled substance with intent to deliver.

In December 2024 the Board of Physical Therapy and physical therapist Aimee J. Wallace (PT00008674) entered an agreed order. Wallace must pay a $1,000 fine and participate in a recovery and monitoring program. In April 2023, Wallace consumed alcohol on her lunch before returning to work.

In December 2024 the secretary of health denied the substance use disorder professional trainee license of Kayla Ann Tompkins (CO61053913). In July 2024, Tompkins was convicted of felon in possession of a firearm, a class B felony.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
