State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Grays Harbor County

In February 2025 the secretary of health granted Rylan Daxon Christensen’s (NC61604693) certified nursing assistant license with conditions. Christensen must enroll in a recovery and monitoring program. In January 2023 and June 2023, Christensen operated a motor vehicle under the influence of alcohol.

King County

In February 2025 the secretary of health notified Xingming Wang of its intent to issue a cease-and-desist order for practicing as a massage therapist without holding a license. In August 2023, Wang performed massage therapy on a patient.

In January 2025 the secretary of health denied the substance use disorder professional trainee license of Lavette Shanell Ford (CO60351583). In June 2017 and January 2018, Ford was convicted of no contact/protection order violation – DV, a gross misdemeanor. In January 2018, Ford was also convicted of second-degree unlawful possession of a firearm, a class C felony.

Pierce County

In February 2025 the secretary of health notified Angelina Tsimberov of its intent to issue a cease-and-desist order for practicing as a behavior technician without holding a license. From July 2024 to August 2024, Tsimberov advertised services as a certified behavioral therapist.

In February 2025 the secretary of health granted Stephen Llewelyn Rousseau’s (CO61620985) substance use disorder professional trainee license with conditions. The license is on probation for four years. During this time, Rousseau’s health care supervisor must submit quarterly performance evaluation reports. Rousseau responded “yes” to questions regarding the use of controlled and chemical substances in the certification application.

Thurston County

In February 2025 the secretary of health granted Mauricio Matias Retambay’s (CG61497618) agency affiliated counselor license with conditions. Retambay must pay $250 cost recovery and complete eight hours of continuing education in patient rights and use of force. In April 2022, Retambay physically abused a patient causing injury to their face.

In February 2025 the secretary of health and agency affiliated registered counselor and independent clinical social worker associate Jonathan Mark Hoyer (CG61114074, SC61124149) entered an agreed order. Hoyer must enroll in a recovery and monitoring program and pay a $1,500 fine. Hoyer responded to the Department of Health regarding complaints on his job performance by reporting he had experienced a substance use relapse.

Yakima County

In February 2025 the secretary of health granted Heather Denise Castaneda’s (CO61642992) substance use disorder professional trainee license with conditions. The license is on probation for three years. During this time, Castaneda’s supervisor must submit quarterly performance evaluation reports. In August 2015, Castaneda was convicted of second-degree theft, a class C felony and driving under the influence, a gross misdemeanor.

Out of State

New Mexico: In February 2025 the secretary of health ordered Nancy Cetlin to cease and desist from practicing psychology until she obtains an appropriate license or meets an exemption. Cetlin must pay a $1,000 fine. Between January 2023 and June 2023, Cetlin provided telehealth psychotherapy services without holding a credential to practice as a psychologist.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
