OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Grays Harbor County
In February 2025 the secretary of health notified Tracy Lynn Bodnar (NA60130391) of its intent to issue a cease-and-desist order for practicing as a registered nursing assistant with an expired license. Between July 2023 and August 2023, Bodnar performed delegated nursing tasks on a patient.
King County
In February 2025 the secretary of health charged registered nursing assistant Mary M. Harder (NA00185335) with unprofessional conduct. In February 2023, Harder allegedly shared confidential patient information and gave a bottle of expired medication to a patient it was not originally prescribed to.
In January 2025 the Dental Quality Assurance Commission denied the dental assistant license of Siwatu Wright (D161621241). In August 2019, Wright pled guilty to felony theft. In January 2016, Wright pled guilty to grand theft.
Kitsap County
In October 2024 the secretary of health charged massage therapist Mitchell Dean Hettinger (MA60868060) with unprofessional conduct. Alleged facts state that in July 2023, Hettinger massaged a patient’s breasts and groin without completing training required to perform breast massage. Hettinger also allegedly initiated a sexual relationship with the patient.
Pierce County
In February 2025 the Chiropractic Quality Assurance Commission charged chiropractor Alexx Delta (CH60691394) with unprofessional conduct. From August 2022 to January 2024, Delta practiced as a chiropractor and provided treatment services with an expired license.
In January 2025 the secretary of health ordered Jennifer Diana Ortiz to cease and desist from practicing massage therapy until they obtain an appropriate license or meet an exemption. Ortiz must also pay a $1,000 fine. From June 2020 to November 2023, Ortiz practiced massage with an expired license.
Skagit County
In February 2025 the Veterinary Board of Governors released Lawrence James Campbell (VT00001774) from the terms and conditions on their veterinarian license.
Stevens County
In February 2025 the secretary of health granted Christina Yumiko Stivers’ (CO61633416) substance use disorder professional trainee license with conditions. The license is on probation until at least June 23, 2026. During this time, Stivers’ health care supervisor must submit quarterly performance evaluation reports. In January 2021, Stivers was convicted of possession of a controlled substance, a misdemeanor. In June 2020, Stivers was convicted of felony possession with intent to deliver a schedule I controlled substance, a felony. In September 2019, Stivers was convicted of a DUI: controlled substance – schedule II or III – 1st offense and possession of a controlled substance, both misdemeanors.
In March 2025 the secretary of health granted Carol Elizabeth Jackson’s (CG61581853) agency affiliated counselor license with conditions. The license is on probation for at least three years. During this time, Jackson’s health care supervisor must submit quarterly performance evaluation reports. In August 2022, Jackson pled guilty to retail theft.
Yakima County
In February 2025 the Pharmacy Quality Assurance Commission charged pharmacy technician and pharmacy assistant Evangelina Corona (VB60925813, VA61161282) with unprofessional conduct. From November 2021 to May 2022, Corona stole over-the-counter medication on more than one occasion from her former place of employment.
Out of State
Oregon: In January 2025 the secretary of health denied the marriage and family therapist license of Sarah Beaulieu (LF61522075). In October 2024, Beaulieu entered a stipulated order with the state of Oregon to surrender their license to practice as a licensed marriage and family therapist.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
