OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Benton County
In March 2025 the Chiropractic Quality Assurance Commission terminated the probation on the chiropractor license of Nathan Scott Anderson (CH60440427).
Clark County
In March 2025 the secretary of health charged certified nursing assistant Christine Ann Turi (NC60283681) with unprofessional conduct. In June 2024, Turi allegedly failed to follow a resident’s care plan resulting in the resident sustaining a fractured femur.
Cowlitz County
In March 2025 the secretary of health charged agency affiliated counselor Krista Lynn Shipman (CG61387131) with unprofessional conduct. In February 2024, Shipman allegedly failed to maintain patient notes for 42 patients.
King County
In March 2025 the secretary of health and mental health counselor Anca Maria Boudreaux (LH60578269) entered an agreed order. Boudreaux’s mental health counselor license is suspended until successful completion of the professional/problem-based ethics course program. Prior to petitioning for reinstatement, Boudreaux must pass the ethics course program. Boudreaux violated client-provider relationships by engaging in a romantic relationship with a client and providing a client with a $4,500 personal loan.
In March 2025 the Dental Quality Assurance Commission reinstated Thi D. Nguyen’s (DE00009351, CS10000438) dental license and license to practice moderate sedation with parenteral agents with conditions. Nguyen must pay a $25,000 fine, reimburse $10,000 in expenses, allow four unannounced audits of enteral and parenteral sedation cases per year for two years, and complete 20 hours of continuing education in medical consultations and treatment of patients with multiple comorbidities. Nguyen must also complete an ethics and boundaries examination, attend two sessions of the American Dental Society of Anesthesiology, and pass one SIM MAN course.
Lewis County
In March 2025 the secretary of health terminated the probation on the certified nursing assistant license of Anna Marie Kooi (NC60290524).
Pierce County
In March 2025 the secretary of health ordered Robert McCrossin to cease and desist from practicing veterinary medicine until they obtain an appropriate license or meet an exception. McCrossin must pay a $1,000 civil fine. Between September 2023 and October 2023, McCrossin administered numerous Bordetella vaccinations to animals without the supervision or authorization of a veterinarian or veterinarian technician.
In March 2025 the secretary of health charged agency affiliated counselor Yourhighness J. Bolar (CG61418230) with unprofessional conduct. In May 2024, Bolar was convicted of second-degree murder, a class A felony, and first-degree unlawful possession of a firearm, a class B felony.
Spokane County
In March 2025 the secretary of health terminated the probation on the registered nursing assistant license of Lindsey Ranee Ott (NA61553243).
In March 2025 the secretary of health terminated the probation on the substance use disorder professional and substance use disorder professional trainee licenses of Brandon John Search (CP61523090, CO61236546).
In March 2025 the secretary of health notified Fangying Zhang of its intent to issue a cease-and-desist order for practicing as a massage therapist without holding a license. In May 2024, Zhang admitted to performing massages without a valid license.
Thurston County
In November 2024 the secretary of health ordered Sarah Marie Sawyer to cease and desist from practicing as a certified medical assistant until they obtain an appropriate license or meet an exception. Sawyer must pay a $1,000 civil fine. Between October 2020 to July 2021, Sawyer performed duties that required a medical assistant certification without holding a license.
In March 2025 the secretary of health charged certified nursing assistant Cynthia M. Pipes (NC60341132) with unprofessional conduct. In June 2023, Pipes allegedly tested positive for marijuana and methamphetamines. Alleged facts also state that Pipes failed to respond to a letter of cooperation from June 2025.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
