OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
King County
In March 2025 the secretary of health charged certified nursing assistant Jessica Anne Borland (NC61125713) with unprofessional conduct. In September 2024, Borland allegedly used a client’s credit card to make personal purchases. She failed to respond in writing to the department’s November 2024 and January 2025 requests for information.
In March 2025 the secretary of health and Anca Maria Boudreaux (LH60578269) entered an agreed order to reinstate her mental health counselor license with conditions. Her license is on probation for at least three years, during which time she may not practice independently. She must submit quarterly unemployment or employer reports, disclose the agreed order terms to all patients at their first visit, allow up to four audits of patient records per year, and pay a $1,000 fine.
In March 2025 the secretary of health granted Kenneth Lance Van Kooten’s (CG61639072) agency affiliated counselor license with conditions. Van Kooten’s license is on probation for at least four years, during which time Van Kooten’s health care supervisor must submit quarterly performance evaluation reports.
Snohomish County
In March 2025 the secretary of health withdrew the notice of intent to issue a cease-and-desist order against the mental health counselor license of Mahshid Babazadeh (LH61048472).
Yakima County
In March 2025 the secretary of health reinstated Mercedes Ambrielle Boyakin’s (NA61189931) registered nursing assistant license with conditions. Boyakin’s license is on probation for at least 12 months, during which time her health care employer must submit quarterly performance evaluation reports.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
