OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In March 2025 the secretary of health and Rosalie Morris (MA61582809) entered an agreed order to reinstate her massage therapy license with conditions. Her license is on probation for at least three years, during which time she must enroll in a department-approved substance use treatment program, submit quarterly progress reports, and comply with random drug testing.
Clallam County
In February 2025 the secretary of health denied Donald Leon Breazeale’s (HM61498178) application for a home care aide credential. In October 2024, Breazeale was charged in Clallam County with two counts of second-degree rape of a vulnerable adult, both class A felonies.
In February 2025 the secretary of health denied Casey Allen Quaadman’s (CG61623822) application for an agency affiliated counselor credential. In February 2022, Quaadman was convicted in Jefferson County of two class B felonies: possession with intent to deliver methamphetamine and possession with intent to deliver heroin.
Clark County
In March 2025 the secretary of health charged agency affiliated counselor Caitlin Christina Crenshaw (CG60937391) with unprofessional conduct. In July 2023, Crenshaw’s employer reported concerns to the department about improper billing and patient exploitation. The respondent has not replied to the department’s June 2024 request for information.
In February 2025 the secretary of health denied Dawnia Rose Jessica Kelley’s (CG61521911) application for an agency affiliated counselor credential. In March 2024, Columbia River Mental Health Services filed a complaint alleging Kelley failed to maintain professional boundaries and safety standards with clients. The department requested more information in May 2024, but Kelley did not reply or respond to follow-up attempts.
Cowlitz County
In March 2025 the secretary of health charged certified nursing assistant DaAngelo Hyde (NC61542352) with unprofessional conduct. In September 2024, Hyde was suspended for allegedly not following the requirements in a patient’s care plan. He also failed to respond to the department’s October 2024 and November 2024 requests for information.
King County
In February 2025 the secretary of health placed the massage therapy license of Xianjun Meng (MA60434382) on probation for three years with conditions. Meng must submit quarterly personal progress reports, complete a record-keeping course within six months, and comply with four unannounced audits per year. Meng was also fined $2,000.
Kitsap County
In April 2025 the secretary of health terminated the probation on the mental health counselor license of Tara Kathryn Overstreet (LH60393078).
Pierce County
In March 2025 the secretary of health and mental health counselor Jodie Ann Leer (LH60160821) entered an agreed order. In September 2024, Leer was charged with unprofessional conduct for failing to report or address staff concerns about employee drug use while working as a supervisor. She must pay a $1,000 fine and complete six hours of continuing education in ethics and mandatory reporting within 30 days.
Skagit County
In April 2025 the secretary of health terminated the probation on the substance use disorder professional trainee license of Shiann Jean Reed (CO61221434).
Snohomish County
In March 2025 the secretary of health charged certified behavior technician Casie B. Rivor (CB61130840) with unprofessional conduct. From early 2022 to November 2023, Rivor engaged in a sexual relationship with a minor, even after being informed the individual was under the age of consent in California.
Spokane County
In March 2025 the secretary of health charged agency affiliated counselor Steffeni R. Crowley (CG61215536) with unprofessional conduct. In December 2022, Crowley began working as a peer support specialist and later failed to maintain professional boundaries in that role. In September and October 2023, the department requested information from Crowley, and despite receiving multiple extensions, she has not responded.
In April 2025 the secretary of health terminated the probation on the agency affiliate counselor license of Leah Michelle King (CG61281619).
Out of State
Florida: In April 2025 the secretary of health charged mental health counselor associate and registered affiliated counselor Kerri Michelle Bicskei (CG61076906, MC61119262) with unprofessional conduct. In February 2023, the department received two complaints about discrepancies between bills submitted by Bicskei and services provided to clients. In May 2023 a records review revealed numerous recordkeeping inconsistencies.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
