State revokes, suspends licenses, certifications, registrations of health care providers-06-06-2025

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Snohomish County

In April 2025 the Department of Health revoked Romulo Ramos De Guzman’s (HM61053327) home care aide credential. The revocation followed De Guzman’s January 2025 felony conviction for second-degree rape, which disqualifies him from working with vulnerable persons under Washington law.

Thurston County

In May 2025 the Department of Health indefinitely suspended Tiffany Renee Holman’s (NA61232077) registered nursing assistant credential. In 2023, the Department of Social and Health Services found Holman had financially exploited a vulnerable adult and placed her on the state’s long-term care abuse and neglect registry, which prohibits Holman from working with vulnerable adults.

Yakima County

In April 2025 the Department of Health and Esther E. Valle (NA00161814) entered an agreed order which suspended Valle’s registered nursing assistant credential for three years. If reinstated, Valle’s credential will be placed on probation with conditions, including working under an approved supervisor and submitting regular performance reports. Between April and December 2023, Valle took advantage of her position for improper financial benefit.

Out of State

Idaho: In May 2025 the Board of Physical Therapy and Peter Jon Faletto (PT00009476) entered an agreed order which suspended Faletto’s physical therapist credential for a minimum of six months. If reinstated, the credential will be placed on probation for at least 30 months. During probation, Faletto must follow several conditions, including undergoing monitoring, providing disclosures, completing ethics education, and allowing unannounced audits. Faletto engaged in inappropriate sexual contact with a patient during and outside of treatment sessions in 2023.

Illinois: In May 2025 the Department of Health summarily suspended Johnshia Juetaun Chatman’s (NC60315605) credential to practice as a certified nursing assistant, pending further disciplinary action. The suspension was based on a Department of Social and Health Services’s finding that Chatman financially exploited and neglected a vulnerable adult in February 2025.
