State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In August 2024 the secretary of health suspended the agency affiliated counselor license of DeAnthony Jamar Phillips (CG61256678) for one year. Prior to petitioning for reinstatement, Phillips must pay a $2,000 fine and complete an in-person professional boundaries course. In June 2023, Phillips provided a patient with his urine for $50 to avoid the patient testing positive on a toxicology screening test. Phillips admitted to providing the patient with urine in exchange for money on a separate occasion, as well as providing a different client with urine while employed at a past treatment center. Phillips failed to respond to a statement of charges by June 2024.

In August 2024 the secretary of health indefinitely suspended the agency affiliated counselor license of Garrett John Thomas Snell (CG61393033). Prior to petitioning for reinstatement, Snell must complete six hours of continuing education and participate in a monitoring program. In February 2023, Snell invited a patient to his hotel room and admitted to relapsing and consuming alcohol. Snell failed to respond to a statement of charges by June 2024.

King County

In August 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse license of Angeliki Joanna Themelis (RN60324151) pending further legal action. In March 2024, the Oregon State Board of Nursing accepted the surrender of Themelis’ credential to practice as a registered nurse. Themelis tested positive for alcohol in a for-cause drug screen while working as a travel nurse.

Yakima County

In August 2024 the secretary of health suspended the substance use disorder professional trainee license of Brittany Roy (CO61169359) for five years. Prior to petitioning for reinstatement, Roy must undergo a complete psycho-sexual evaluation and demonstrate that she meets relevant regulatory requirements for a credential. In November 2023, Roy was arrested on grounds of communicating with a minor patient for immoral purposes and for sending sexually suggestive messages.
