OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Clallam County
In May 2023 the secretary of health indefinitely suspended the agency affiliated counselor license of Joshua Wayne Reed (CG61087528). Reed did not comply with a November 2020 agreement to submit quarterly performance evaluation reports.
Clark County
In April 2023 the Dental Commission indefinitely suspended the dental assistant license of Kristina Velasquez Mendoza (D160669663). In March 2021, Mendoza called prescriptions into a pharmacy for her boyfriend, using two dentists’ names without their authorization.
Jefferson County
In May 2023 the Veterinary Board of Governors indefinitely suspended the veterinarian license of Madelyn Rose Curll (VT00003121). Curll may petition for reinstatement if she submits to a neurocognitive and neurobehavioral evaluation by a Board-approved evaluator. Curll was found to suffer from a health condition that impairs her ability to practice with reasonable skill and safety.
Kitsap County
In May 2023 the secretary of health revoked the home care aide license of Nova Maria Morton (HM60487359). In December 2022, Morton was placed on the Department of Social and Health Services list as a person who is disqualified from caring for vulnerable adults, juveniles, and children. In April 2022, Morton kicked a vulnerable adult in her care in the groin.
Pierce County
In May 2023 the secretary of health denied the home care aid credential of Janae A Jones (HM61290219). In November 2022, Jones was placed on the Department of Social and Health Services registry as a person disqualified from caring for vulnerable adults, juveniles, and children for financially exploiting a vulnerable adult.
Skagit County
In May 2023 the secretary of health indefinitely suspended the substance use disorder professional license of Tara L Brinson (CP00006297). Brinson failed to comply with a November 2022 agreement that she contact and enroll in a substance use monitoring program.
Out of State
Arizona: In May 2023 the Nursing Commission summarily suspended the registered nurse license of Julie Dawn Ryan (RN61184145). In October 2022, Ryan surrendered her license in Arizona after failing to comply with terms and conditions of probation from a previous order.
Florida: In May 2023 the Nursing Commission indefinitely suspended the registered nurse license of Nilka Diaz (RN61193297). In December 2022, the California Board of Registered Nursing revoked Diaz’s license based on findings she was grossly negligent and incompetent in her care of an ICU patient.
Idaho: In May 2023 the Pharmacy Commission summarily suspended the pharmacist license of John Ryan Bronsell (PH61218022). In April 2022, Bronsell’s license was suspended in Idaho after he was indicted for two counts of “acquiring or obtaining a controlled substance by misrepresentation, fraud, forgery, deception and/or subterfuge.”
