State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2023 the Nursing Commission indefinitely suspended the registered nurse license of Joseph Karl Wiser (RN60635525). In February 2021, Wiser was convicted of driving under the influence. In March 2022, Wiser was convicted of third-degree malicious mischief. Wiser may petition for reinstatement after undergoing a neuro-psychological evaluation by a Commission-approved licensed clinical neuro-psychologist, and after actively participating in the intake process with a substance use monitoring program.

In June 2023 the Nursing Commission indefinitely suspended the licensed practical nurse credential of Michael Ray McCarter (LP60832138). In August 2022, the Oregon State Board of Nursing accepted McCarter’s voluntary surrender of his credential after McCarter engaged in unacceptable behavior towards co-workers. McCarter may petition for reinstatement after undergoing a psychological and/or psychiatric evaluation by a Commission-approved board-certified psychiatrist.

Cowlitz County

In June 2023 the Nursing Assistant Program and Amanda Michelle Cobb (NA60560947) agreed to the suspension of her registered nursing assistant license. Cobb may not petition for reinstatement for at least 24 months and prior to reinstatement must pay a $250 fine, and complete four hours of continuing education in the areas of patient rights, and law and ethics. In January 2023, Cobb was placed on the Department of Social and Health Services’ Adult Protective Services long-term care abuse and neglect registry.

Spokane County

In June 2023 the secretary of health summarily suspended the certified nursing assistant and registered medical assistant licenses of Nicole Ann Chamberlin (NC61158977, MR61355884) pending further legal action. In December 2022, Chamberlin was placed on the Department of Social and Health Services’ Adult Protective Services long-term care, abuse and neglect registry and is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

In June 2023 the secretary of health suspended the certified behavior technician license of Savannah Jane Johnson (CB61287065). Johnson may not petition for reinstatement for at least five years and prior to reinstatement must pay a $5,000 fine. In July 2022, Johnson took at least $2,000 from a patient’s mother’s wallet.

Thurston County

In June 2023 the Examining Board of Psychology indefinitely suspended the psychologist license of Gregory L. Williams (PY00002410). Williams may petition for reinstatement. In March 2022, Williams failed to provide a patient’s treatment records to them and their attorney. Williams has since not responded to numerous Psychology Board and Department of Health attempts to contact him.

Out of State

Hawaii: In June 2023 the Nursing Commission indefinitely suspended the licensed practical nurse credential of Ashia Lray Jones (LP60955825). Jones may petition for reinstatement. In March 2023, Jones relinquished her license in Colorado after failing to comply with an agreement she entered into with the State of Colorado Board of Nursing that required that she comply with a Peer Assistance Services contract.

Oregon: In June 2023 the Massage Therapy Program and Qingguang Li (MA60996962) agreed to the voluntary surrender of his massage therapist license. Li agrees to never practice massage in the state of Washington again and has no right to reapply. In October 2021, Li touched a patient’s breast and genitalia without consent.
