State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2023 the secretary of health revoked the home care aide license of Katrice Latara Lemons (HM60832083). In June 2021, Lemons stole a patient’s Oxycodone medication and financially exploited a vulnerable adult.

Franklin County

In June 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Elvia G. Lothrop (RN60023184). Lothrop failed to comply with the terms and conditions of a May 2023 agreement where she was to enroll and participate in a monitoring program if offered a contract. Lothrop failed to comply with her monitoring contract and was terminated from the program.

Grays Harbor County

In July 2023 the secretary of health indefinitely suspended the registered nursing assistant license of Kerri Kennedy Layton (NA61006977). Layton may file a petition for reconsideration. Layton failed to respond to a statement of charges and request for settlement and hearing sent in May 2023. In January 2023, Layton was placed on the Department of Social and Health Services’ Adult Protective Services long-term care abuse and neglect registry.

Kittitas County

In June 2023 the Board of Osteopathic Medicine and Surgery and Anna Elperin (OP60576478) agreed to permanently restrict her osteopathic physician and surgeon license. Elperin may not issue COVID-19 vaccination waivers and she may not conduct Department of Transportation medical examinations. Elperin must complete six hours of continuing education and will have up to 10 patient records audited up to four times a year for three years. She must also pay a fine of $5,000.

Mason County

In July 2023 the Nursing Assistant Program and Christina Lynn Goers (NC60726732) agreed to the suspension of her certified nursing assistant license. Goers may not petition for reinstatement for at least 24 months. Prior to reinstatement, she must pay a $500 fine and complete eight hours of continuing education in the areas of patient boundaries and law and ethics.

Pacific County

In July 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Sandra Kay Donato (RN00116820). In April 2022, Donato failed to comply with a monitoring contract by withdrawing from the monitoring program after a positive drug screen and positive urine alcohol tests.

Pierce County

In June 2023 Dental Quality Assurance Commission indefinitely suspended the dental assistant license of Jasmine Lavina Naniloa Kananiokahome Caminos (D160575758). Caminos may file a petition for reconsideration. Caminos failed to comply with a 2022 agreed order by withdrawing from a monitoring program without permission and failed to successfully complete the program.

In July 2023 the secretary of health indefinitely suspended the registered nursing assistant license of Antony Ngigi Mwaura (NA61175902). Mwaura may petition for reinstatement. Mwaura has not responded to numerous Department of Health attempts to contact him to address multiple allegations including consuming alcohol while working at an adult family home.

Spokane County

In June 2023 the secretary of health revoked the home care aide license of George Patrick Sessions (HM60953773). Sessions violently assaulted a patient, a vulnerable adult, including kicking him and throwing him to the ground. The patient suffered severe injuries leading to his death in which the medical examiner ruled a homicide.

Thurston County

In July 2023 the secretary of health indefinitely suspended the home care aide license of Rose Marie Kinder (HM60803022). Kinder may petition for reinstatement. Kinder has not responded to numerous Department of Health attempts to contact her to address multiple allegations including testing positive for methamphetamines in a drug screen mandated by her employer in June 2022.

Out of State

Idaho: In July 2023 the Pharmacy Quality Assurance Commission indefinitely suspended the pharmacist license of Benjamin Albert Gage (PH60560940). Gage may file a petition for reconsideration. Gage failed to respond to a consent order from the Idaho Board of Pharmacy that placed conditions on Gage’s license. The conditions included, but were not limited to, completing a drug and alcohol assessment and completing any treatment recommendations.

Texas: In July 2023 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse license of Tatiana Maria Burke (RN60936657) pending further legal action. The Oklahoma Board of Nursing’s findings of Burke's unprofessional conduct included inaccurate recording, falsifying or altering patient records; conduct detrimental to the public interest; jeopardizing the life, health, or safety of a patient by the unauthorized altering of medications, failing to utilize appropriate judgement in administering safe nursing practice or patient care; and failing to exercise technical competence in carrying out nursing or patient care.
