OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
King County
In June 2023 the secretary of health revoked the home care aide license of Geoffrey Kuira Kabiru (HM60718268). In June 2021, Kabiru was convicted of third-degree child molestation, a felony.
In July 2023 the secretary of health indefinitely suspended the marriage and family therapist license of Pamela Kujawsky Kendrick (LF00001552). Kendrick may petition for reinstatement. Kendrick has not responded to numerous Department of Health attempts to contact her to address a complaint brought against her in January 2022.
Spokane County
In July 2023 the Substance Use Disorder Professional Program and Darcie June Crandall (CP60080422) agreed to the voluntary surrender of her substance use disorder professional license. Crandall agrees to never practice substance use disorder counseling in the state of Washington again and has no right to reapply. In June 2021, Crandall fraudulently documented the duration of telephone services and lied to her supervisor about providing telephone services to clients.
Whitman County
In July 2023 the secretary of health indefinitely suspended the certified nursing assistant license of Nicholas Tinina Kayioni (NC60411462). Kayioni did not respond to a letter of cooperation issued December 2022 or a statement of charges issued May 2023. In September 2019 a four-year probation became effective in Idaho on Kayioni’s certified nursing assistant license after he was convicted of possession of a controlled substance, a felony. Kayioni failed to notify the Washington Department of Health of the 2019 Idaho Order.
Out of State
Idaho: In July 2023 the Pharmacy Quality Assurance Commission indefinitely suspended the pharmacist license of Jill Dawn James (PH60462795). James may file a petition for reinstatement. James failed to respond to a statement of charges sent April 2023. James had her Idaho license revoked in May 2018 and her Colorado license revoked in February 2019.
