State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Cowlitz County

In October 2023 the Board of Nursing (formerly the Nursing Commission) denied the application to reactivate the registered nurse license of Melissa L. Gibson (RN00160154). Gibson’s license is suspended, and she may not petition for reinstatement for at least 60 months. Prior to petitioning for reinstatement, Gibson must undergo a psychological and/or psychiatric evaluation and she must contact a monitoring program and, if required, enroll in the monitoring program. In 2016, Gibson was denied a registered nurse license in Oregon after finding that in 2014, Gibson was convicted in Washington state for disorderly conduct-domestic violence and obstructing a law enforcement officer. Then, in 2018, Gibson was convicted in Oregon of menacing, unlawful use of a weapon and attempting to commit a felony.

King County

In October 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Michelle Renee Sixkiller (RN60441517). Sixkiller may petition for reinstatement. In April 2023, Sixkiller withdrew herself from a 5-year monitoring contract entered into in January 2020.

Pierce County

In October 2023 the Board of Nursing (formerly the Nursing Commission) summarily suspended the licensed practical nurse license of Shelley Danielle Randall (LP61309550) pending further disciplinary proceedings. In March 2023, Randall’s Louisiana practical nurse license was suspended by the Louisiana State Board of Practical Nurse Examiners for unprofessional conduct including falsifying records, being convicted of a crime which reflects the inability to practice practical nursing with due regard to the health and safety of patients, and for being under the influence of drugs and alcohol while on duty.

Spokane County

In October 2023 the secretary of health indefinitely suspended the certified nursing assistant license of Theresa Lynn Mowatt (NC10010047). Prior to petitioning for reinstatement, Mowatt must contact a monitoring program and, if required, enroll in the monitoring program. In July 2022, Mowatt smelled of alcohol when she arrived at her shift at a supported living home for vulnerable adults.
