OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
King County
In January 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Miryah Lynn Petroff (RN00173873). Prior to petitioning for reinstatement, Petroff must re-enter a monitoring program. In September 2023, Petroff failed to comply with a 2021 monitoring contract due to lack of program participation and a missed test.
In January 2024 the secretary of health suspended the massage therapist license of Jeremy Hatch (MA61004273). Hatch may not petition for reinstatement for at least five years. Prior to petitioning for reinstatement, Hatch must undergo a complete psycho-sexual evaluation. In August 2022, Hatch entered a Stipulation of Informal Disposition (STID) with the Department of Health and failed to comply with the terms and conditions by continuing to display a pattern of sexual misconduct against clients.
Whatcom County
In January 2024 the Board of Osteopathic Medicine and Surgery and Gregory D. Sharp (OP00001611) agreed to the voluntary surrender of his osteopathic physician and surgeon license. Sharp agrees to never resume the practice of osteopathic medicine and surgery in the state of Washington. Sharp's treatment of patients fell beneath the standard of care regarding management of scheduled medications and the practice of pain management.
Out of State
Florida: In January 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Mary Jane Cardillo (RN60970745). Prior to petitioning for reinstatement, Cardillo must contact a monitoring program and, if required, enroll in the monitoring program. The California Board of Registered Nursing issued an order effective in January 2023 that placed Cardillo’s license on probation for three years with certain terms and conditions including, but not limited to, participation in a treatment program.
Georgia: In January 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Imani Shakari Sabria Askin (RN61241513). Prior to petitioning for reinstatement, Askin must contact a monitoring program and, if required, enroll in the monitoring program. In April 2023, the Georgia Board of Nursing placed Askin’s license on probation, subject to terms and conditions, and restricted Askin from accessing controlled substances for two years.
Texas: In January 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Taylor Ashley Porter (RN60781446). Prior to petitioning for reinstatement, Porter must contact a monitoring program and, if required, enroll in the monitoring program. In January 2023, the Texas Board of Nursing placed Porter’s Texas registered nurse license on probation for a minimum of three years. The action was based on positive drug screens and findings of alcohol and/or controlled substance use while subject to call or duty.
