OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In February 2025 the secretary of health and Casandra Maria Chavez (NC60726065) agreed to conditions that indefinitely suspend Chavez’s certified nursing assistant license. Prior to reinstatement, Chavez must pay a $250 fine, complete 10 hours of continuing education in law and ethics, patient rights and boundaries, and respond to the letter of cooperation. In April 2024, Chavez purchased marijuana and items from an adult sex store for personal use while a patient waited in the car. Chavez also failed to respond to a letter of cooperation from August 2024.
Clark County
In February 2025 the secretary of health suspended Aubrey Krystal R. Schnell’s (HM61006268, NA61333794) home care aide and registered nursing assistant licenses for at least three years. Prior to petitioning for reinstatement, Schnell must complete 12 hours of continuing education in law and ethics and boundaries and pay a $250 fine. Between July 2015 and June 2022, Schnell assisted a patient in shoplifting multiple times and engaged in inappropriate sexual conduct that caused emotional distress to a vulnerable adult patient. Schnell also failed to respond to a statement of charges from October 2024.
Skagit County
In February 2025 the secretary of health and Justin Loren Dean (NC60521307) agreed to conditions that suspend Dean’s certified nursing assistant license for at least six years. Prior to petitioning for reinstatement, Dean must pay a $1,000 fine and continue to comply with conditions on an August 2024 judgement and sentence. In July 2024, Dean was convicted of indecent liberties-mentally incapacitated/physically helpless, a class B felony and third-degree rape-lack of consent, a class C felony.
Thurston County
In January 2025 the secretary of health suspended Timothy J. Cosden’s (MA60071009) massage therapist license for at least 10 years. Prior to petitioning for reinstatement, Cosden must pay a $5,000 fine and undergo a psycho-sexual evaluation. In March 2022, a criminal complaint was filed against Cosden for fourth-degree assault with sexual motivation, a gross misdemeanor.
