State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

King County

In August 2018 the Home Care Aide Program indefinitely suspended the home care aide credential of Elizabeth Stella Gebremedhin (HM60731565), who didn’t comply with a requirement to participate in a substance abuse monitoring program.

Pacific County

In September 2018 the secretary of health entered an agreement with chemical dependency professional Jennifer Marie Harris (CP60450956) that suspends her credential for at least three years. Harris had an intimate relationship, including sexual contact, with a former client.

Pierce County

In August 2018 the secretary of health suspended for at least five years the massage therapist credential of Eric Keith Young (MA60681717). In 2018 he was charged with two counts of indecent liberties in connection with allegedly non-therapeutic contact with a patient.

San Juan County

In August 2018 the secretary of health permanently revoked the registered nursing assistant credential of Eric Arthur Kulp (NA60798900). In 2018 Kulp was charged with second-degree murder-domestic violence, and with unlawful imprisonment.

Out of State

Idaho: In June 2018 the Nursing Commission modified an agreement with registered nurse Gwyneth F. Faye, also known as Gwyneth F. Hafstad (RN00148594), to indefinitely suspend her credential. In May 2017 the Nursing Commission agreed to reinstate Faye’s credential and placed her on probation with practice limitations. Faye’s license was suspended in 2015. In 2014 she was charged in Washington with six counts of second-degree assault in connection with pointing a firearm at six people.

Texas: In August 2018 the Nursing Commission indefinitely suspended the registered nurse credential of Melanie Joan Ludig (RN60481439). Ludig’s Texas registered nurse credential was suspended in 2018.

Virginia: In August 2018 the Nursing Commission indefinitely suspended the registered nurse credential of Brett Lars Crawford (RN60651585). In 2017 Crawford surrendered his California registered nurse license.
