OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Chelan County
In November 2024 the Department of Health summarily suspended Joshua Daniel Tegarden’s (NC60554665) certified nursing assistant credential pending further disciplinary action. In December 2023, the Department of Social and Health Services’ Adult Protective Services found that Tegarden financially exploited a vulnerable adult. The finding became final that month, and he was placed on the state’s long-term care abuse and neglect registry, which prohibits employment involving unsupervised access to vulnerable adults.
Clark County
In April 2025 the Department of Health summarily suspended Gerrianne Aguon’s (NC10098955) certified nursing assistant credential pending further disciplinary action. In July 2024, the Department of Social and Health Services’ Adult Protective Services found Aguon likely financially exploited a vulnerable adult and placed her on the state’s long-term care abuse and neglect registry, barring her from roles with unsupervised access to vulnerable adults.
Columbia County
In November 2024 the Department of Health indefinitely suspended Tonde Rowlette’s (CO61505554) substance use disorder counselor trainee credential pending compliance with an ongoing investigation. In February 2024, Rowlette tested positive for illegal substances during a pre-employment drug screening. The department requested a response in May 2024, but as of July 2024, Rowlette had not replied, despite follow-up attempts.
Skagit County
In November 2024 the Department of Health suspended Dell James Campbell’s (CG60943646) agency affiliated counselor license for three years. Campbell must wait the full term, pay a $3,000 fine, and complete a professional ethics course before petitioning for reinstatement. If reinstated, Campbell’s license will be placed on probation for at least two years. During that time, he must be supervised by a qualified professional with no prior relationship and at least five years of counseling experience. The supervisor must submit regular reports to the department. Additionally, Campbell is required to disclose the disciplinary order to all patients before their first visit, and to all current and future health care employers, and he must permit up to three unannounced audits to ensure compliance with probation terms. In 2023, Campbell engaged in a sexual relationship with a former patient after her discharge. He later admitted to the conduct and accepted responsibility.
Snohomish County
In April 2025 the Pharmacy Quality Assurance Commission denied Julia A. Barnett’s (IR61498196) application to register as a pharmacy intern. The decision was based on the indefinite suspension of her physician and surgeon license, as well as concerns about patient safety.
