Apply for Dementia Awareness Grant

If you are a community or faith-based organization in Washington state, we invite you to complete the application for our new grant to promote awareness and education about dementia, including Alzheimer's disease. The deadline to submit the application is 11:59 p.m. on April 4, 2025.

Application Scoring

We hope to fund all applicants who apply who meet the requirements. However, if more applications are received than can be funded, the grant advisory group consisting of community leaders and those who serve highly impacted populations will assist in determining final selection using defined scoring criteria.

The criteria to be considered but not required include:

The committee will attempt to have a mix of awardees from locations across the state, a mix of urban and rural, and a mix of diverse populations served as they decide on the final awardee list. A notification of award will be made by DOH around April 18, 2025.

  • Describe how you would meet the required grant activities of choosing a trusted messenger that will attend the facilitator training and host two Brain Health and Dementia Awareness sessions. Please answer the following questions:
    • Have you already selected a trusted messenger to attend the facilitator training and host the two educational sessions? If so, describe the reasons this person was selected. If not, describe how you will choose your trusted messenger.
    • Describe how you envision hosting the two educational sessions (e.g. preferred location, likely timing).
    • What population will you invite to each of these session (target audience), and how many people do you estimate will attend each of the sessions?
    • What do you hope is the outcome at the end of this grant?
