- Washington State License and Status
- 
	An applicant must have held a health care profession credential issued by the department. That license must be expired but the expiration date must not be longer than 10 years. There must be no restrictions on the applicants ability to activate that license. An applicant is not eligible for a retired volunteer medical worker license if they hold any current health care profession credential issued by the department. 
- Current Volunteer Registration
- 
	Verification of current registration as a volunteer emergency worker with a local organization for emergency services or management. 
- Personal Data Questions
- 
	Applicant must answer personal data questions listed in the application. An appropriate explanation and required documentation must be sent with positive answers. If there is a positive answer to the professional liability claims history question, the applicant must send an explanation of the nature of the case, data and summary of care given, copies of the original complaint and the settlement or final disposition. If pending, applicant must indicate status. 
If the federal government shutdown continues, Supplemental Nutrition Assistance Program (SNAP) benefits in Washington will end on Oct. 31, 2025. DSHS has more information about the impacts as well as what clients can do to prepare for the loss of benefits.
